Mali型假性Kaposi肉瘤并发淤积性皮炎

摘　　要：报告1例Mali型假性Kaposi肉瘤并发淤积性皮炎。患者男,23岁。左小腿紫褐色丘疹及斑块1年余,伴肿胀、溃疡及疼痛2个月,伤口难以愈合。双下肢静脉曲张3年。皮肤科检查:皮损沿静脉曲张部分布,其上覆疣状增生性斑块,皮损周围见散在深褐色瘀斑,足、趾背无损害。皮损组织病理符合假性Kaposi肉瘤。免疫病理:血管内皮细胞CD34强阳性表达,周围间质细胞CD34阴性。诊断为Mali型假性Kaposi肉瘤并发淤积性皮炎。A case of pseudo-Kaposi sarcoma（Mali type） complicated with stasis dermatitis is reported. A 23-year-old male presented with purple-brown papules and plaques on the left leg for over 1 year, accompanied with swelling, ulcer and pain for 2 months. Wounds were hard to heal. Patient had a 3-year history of varicosis on both the lower extremities. Physical examination showed that skin lesions were distributed along varicose veins covered with verrucous hyperplastic plaques, surrounded by scattered dark brown ecchymosis, but no lesions on the foot or dorsal toes. Histopathologic examination of skin lesions was consistent with pseudo-Kaposi sarcoma. Immunohistochemical staining revealed strong CD34-positive in vascular endothelial ceils, but CD34-negative in surrounding mesenchymal cells. Therefore, the diagnosis of pseudo-Kaposi sarcoma （Mali type） complicated with stasis dermatitis was made.
