摘  要:目的探讨Lgr5表达及Kras基因突变与结直肠癌临床病理特征的关系。方法采用免疫组织化学方法,检测539例结直肠癌患者癌组织中Lgr5的表达,应用聚合酶连反应(polymerase chain reaction,PCR)扩增DNA直接测序法检测Kras基因第12、13密码子突变,并分析其与临床病理特征的关系。结果在539例结直肠癌中,Lgr5总体表达率为66.79%(360/539),在不同组织学类型、分化程度及病理分期分组中,其表达差异有统计学意义(P<0.05),与肿瘤分化(γ=0.102,P=0.018)、肿瘤分期(γ=0.093,P=0.030)、浸润深度(γ=0.131,P=0.002)及是否有脉管和神经的侵犯(γ=0.087,P=0.044)呈正相关性;Kras基因突变总突变检出率为37.29%(201/539),Kras基因突变与性别及结肠部位有关(P<0.05),女性及右半结肠的突变率较高。Lgr5表达与Kras基因突变间无相关性。结论 Lgr5表达与肿瘤的发生发展相关,提示Lgr5可能是治疗的潜在靶点。Kras基因突变在女性及右半结肠癌中检出率较高,突变类型与Lgr5表达无显著相关性。Objective To investigate the correlation of Lgr5 expression and Kras gene mutations with clinicopathological features of eolorectal cancer(CRC). Methods The expression of Lgr5 in 539 cases of CRC were detected by immunohistochem- istry. Genomic DNA was extracted and mutations of Kras gene (codon 12 and 13 ) were detected by polymerase chain reaction (PCR). Then to explore their relationship with the clinical pathologic features. Results In 539 cases of CRC, Lgr5 expression rate was 66.79% (360/539). Lgr5 expression had statistical significance in different histological type, differentiation degree and pathalogical staging groups( P 〈 0.05 ). Lgr5 expression was positively correlated with differentiation degree ( T = 0. 102, P = 0.018 ) , patbalogical staging ( T = 0.093, P = 0. 030 ) , infiltration depth ( T = 0.131, P -- 0. 002 ) and whether a vasculai or nerve involvement (T = 0. 087 ,P = 0. 044). The total Kras gene mutation detection rate was 37.29% (201/539). Kras gene mutation was related ~n sex and the site of colon groups. The mutation detection rate of female and the right colon cancer were higher than male and the left colon cancer. There was no correlation between Lgr5 expression and Kras gene mutation. Conclusion Lgr5 ex- pression is related with colorectal carcinoma and may be a potential target for therapy. Kras gene mutation has high detection rate in female and right colon cancer, and has no significant correlation with Lgr5 expression.
