中药辨证施治对胃癌前病变患者Bcl-2、Ki-67和APC蛋白的影响

摘　　要：目的:探讨中药辨证施治对胃癌前病变组织Bcl-2、Ki-67和APC蛋白表达的影响。方法:选取2015年1月1日—2015年12月31日本院收治的36例胃癌前病变患者作为研究对象,按照1:1的比例随机分为中药实验组和西药常规组,分别予中药辨证施治、叶酸口服治疗6个月,并于治疗前、治疗结束时行免疫组化检查,同时另在本院选10例Hp阴性正常胃病理切片作为正常对照,对比分析Bcl-2、Ki-67和APC蛋白表达的变化情况。结果:癌前病变组患者治疗前Bcl-2(41.67%)、Ki67(44.44%)总的阳性率均高于正常对照组(均为0%),APC蛋白总的阳性率(55.56%)低于正常对照组(90.00%),差异具有统计学意义(P<0.05);中药实验组和西药常规组患者治疗前后病理组织Bcl-2、Ki67蛋白免疫组化结果比较,差异有统计学意义(P<0.05)。中药实验组治疗前后病理组织APC蛋白免疫组化结果比较,差异有统计学意义(P<0.05),相反,西药常规组治疗治疗前后病理组织APC蛋白免疫组化结果比较,差异无统计学意义(P>0.05)。结论:Bcl-2、Ki67蛋白在胃癌前病变组织中表达上调,APC蛋白表达下降。中药辨证施治可能通过抑制Bcl-2蛋白异常表达、降低Ki67细胞增殖活性,从而逆转胃癌前病变患者黏膜异型增生、萎缩、肠化。Objective ： To investigate the influence of TCM syndrome differentiation on Bcl - 2, Ki - 67 and APC of patients with lesions before gastric carcinoma. Methods ： Thirty - six cases with lesions before gastric carcinoma in our hospital from January 1,2015 to December 31,2015 were selected as study objects. They were divided into TCM observation group and Western medi- cine common group. They were all given 6 months＇ TCM syndrome differentiation and oral folic acid and immunohistochemical check before and after treatment. Another 10 cases with Hp negative normal pathological section in our hospital were selected as control group. Compare the changes of Bcl - 2, Ki - 67 and APC protein expression. Results ： Total positive rates of Bcl - 2 and Ki67 of NDR （41.67% ,44.44% ） were higher than the control group＇s （0% ,0% ） and the total positive of APC （55.56%） was lower than the control group＇s （90.00%）. The difference was statistically significant （ P 〈 0. 05 ）. Compare the Bcl - 2 and Ki67 of TCM observation group and Western medicine common group. The difference was statistically significant （ P 〈 0. 05 ）. Difference between APC before and after treatment in TCM observation group was statistically significant （ P 〈 0. 05 ）. On the con- trary,the difference between APC before and after treatment in Western medicine common group was not statistically significant （ P 〉 0. 05 ）. Conclusion ： Bcl - 2 and Ki67 protein expressions are up - regulated in pathological tissues before gastric carcinoma and APC protein expression is declined. TCM syndrome differentiation may reverse mucosal hyperplasia, atrophy, intestinal metaplasia by inhibiting Bcl -2 abnormal protein expression and lowering Ki67 cell proliferation activity.
