深在性囊性胃炎7例临床特点分析

摘　　要：目的分析深在性囊性胃炎临床特点。方法回顾性分析2015年1月—2016年12月武汉大学人民医院消化内科诊治深在性囊性胃炎患者7例临床资料,分析其临床症状、内镜及影像学表现、病理学结果等临床资料。结果 7例患者均无胃部手术史,1例既往慢性胃炎及胃溃疡病史,1例幽门螺旋杆菌(Hp)感染史。表现为上腹疼痛3例,腹胀1例,其他3例无典型症状。7例内镜下均表现为单发隆起病灶,分别位于贲门、胃底、胃体及胃窦,其中3例行超声胃镜检查。7例患者影像学检查,2例无特异表现,1例为囊性密度灶,其余均为胃壁增厚。病理学结果除深在性囊性胃炎典型表现外,伴有轻度肠上皮化生4例,伴有低级别上皮内瘤变3例。7例患者中5例行内镜下黏膜剥离术(ESD),2例行外科手术治疗,术后均未发生严重并发症,预后良好。结论深在性囊性胃炎临床症状及内镜表现均不典型,超声内镜结合CT检查有利于疾病诊断,治疗上单发病灶以内镜下治疗为主。Objective To investigate the clinical characteristics in patients with gastritis cystica profunda in order to improve the knowledge about GCP and promote the level of clinical diagnosis and treatment.Methods The clinical data of seven patients with gastritis cystica profunda from department of gastroenterology in Renmin Hospital of Wuhan University from January 2015 to December 2016 were retrospectively reviewed.Results Among the 7 patients with gastritis cystica profunda, 1 case had chronic gastritis and gastric ulcer history, 1 case had Hp infection history.3 cases suffered from abdominal pain, 1 case suffered from bloating, other 3 cases no symptom, respectively.In all cases, there were no histories of operation in stomach.7 cases of gastritis cystica profunda were located in the cardia with 1 case, in the gastric fundus of 1 case, in the gastric body of 1 case, in the gastric antrum of 4 cases, respectively.There were all single lesions.3 cases of gastritis cystica profunda were accepted the examination of ultrasonic gastroscopy.2 cases of gastritis cystica profunda had no special manifestation in CT scan, 1 case of gastritis cystica profunda showed the cystic density focal in CT scan, others showed the thickening of gastric wall.Except for the manifestation of gastritis cystica profunda in the pathology, 4 cases of gastritis cystica profunda combined with mild intestinal metaplasia, 3 cases of gastritis cystica profunda combined with low grade intraepithelial neoplasia.In 5 of 7 cases of patients with endoscopic mucosal dissection （ESD）, 2 underwent surgical treatment, no serious complications during postoperative were found, the prognosis is good.Conclusion The symptoms of gastritis cystica profunda are not typical.The ultrasonic gastroscopy and CT scan may be helpful for the diagnosis of gastritis cystica profunda.The single lesion may be treated with gastroscopy.
