摘　　要：目的:探讨甲状腺肿瘤切除术中冰冻切片检查的应用效果及误漏诊原因。方法:采集246例甲状腺肿瘤患者的病理组织,分别进行冰冻切片检查与石蜡切片检查。结果:经冰冻切片检查,187例被诊断为良性病变,占76.0%;59例被诊断为恶性病变,占24.0%。经石蜡切片检查,182例被诊断为良性病变,占74.0%;64例被诊断为恶性病变,占26.0%。冰冻切片检查中有5例恶性病变被诊断为良性病变,恶性病变的诊断准确率为92.19%。结论:冰冻切片检查方法应用在甲状腺肿瘤切除术中的整体检查准确率较高,但也存在一定的误漏诊情况;造成误漏诊的原因包括获取患者病理组织样本的取材局限性、甲状腺肿瘤本身病变的形态以及病理性质较为复杂、当前的冰冻切片检查技术水平不高、切片样本处理的复杂性与处理时间上的局限性等。Objective： To investigate the application efficacy of frozen section examination in thyroid tumor resection and the reason analysis of misdiagnosis and missed diagnosis. Methods： 246 pathological tissues of patients with thyroid neoplasms were collected and performed with the frozen section examination and the paraffin section examination respectively. Results： The frozen section examination indicated that 187 of them were diagnosed as benign lesions,accounting for 76. 01 %; 59 of them were diagnosed as malignant lesions,accounting for 23. 98 %. While the paraffin section examination demonstrated that 182 of them were diagnosed as benign lesions,accounting for 73. 98 %; 64 of them were diagnosed as malignant lesions,accounting for 26. 02 %. 5 cases of malignant lesions were diagnosed as benign lesions in the frozen section examination,the diagnostic accuracy rate for malignant lesions being 92. 19 %. Conclusion： The frozen section examination has higher accuracy rate in the diagnosis of thyroid tumor resection,but there is also some misdiagnosis and missed diagnosis. The reasons for misdiagnosis and missed diagnosis mainly include limitations to get pathological tissues,complicatedly morphological and pathologic features of thyroid neoplasms themselves,low technological level of the frozen section examination,complexity in samples treatment and limitations in processing time.
