摘 要:目的探讨CT增强扫描在肝癌微波消融治疗术后疗效判定中的价值。方法回顾性分析我院2014年4月至2016年6月在超声引导下行微波消融(microwave ablation,MWA)治疗的60例肝癌患者,定期进行CT增强扫描检查,观察微波消融的效果及肿瘤复发的情况。结果肿瘤残留或原位复发的主要征象是消融区的边缘呈带状或结节样强化,或随访过程中肿瘤体积的不断增大。在微波消融治疗后的首次CT复查中,72个(80%)病灶出现完全坏死。在随后的CT随访复查中,8个消融灶出现肝内原位复发。结论增强CT能有效评价MWA疗效,在其长期随访过程中,能为肿瘤消融术后治疗提供可靠诊断依据。Objective To assess the role of helical CT in the longterm followup of patients with hepatocellular carcinoma treated with MWA. Methods CT findings of 60 patients in our hospital Between April 2014 and June 2016 with hepatocellular carcinoma were reviewed retrospectively, who underwent percutaneous ultrasoundguided MWA. CT images consisted of nonenhanced and The third phase of the scanning, the effect of ablation therapy and tumor recurrence were detected. Results The major patterns The complete necrosis lesions in 72 (80%)were seen on the initial postablation CT scans after a single session MWA. On the subsequent follow- up CT examination, the local intrahepatic tumor recurrences were seen in 8 cases. Conclusion: For certain pa-tients with hepatic malignant tumors, RFA is an effective therapeutic method, and dual-phase contrast enhanced helical CT scans play aimportant role in long- term assessment and the effect of MWA, in the process of its tumor after ablation therapy.
