不同部位脑血管机械取栓治疗并发症差异性的临床研究

摘　　要：目的分析不同部位脑血管机械取栓治疗并发症的发生是否存在差异性,从而进行经验总结,指导临床治疗。方法选取包头市中心医院神经内科收治的行Solitaire AB支架机械取栓治疗的115例急性缺血性脑卒中患者,按照患者的取栓部位,分成MCA、ICA、VA、BA,VA-BA,ICA-MCA,前循环和后循环,对比各部位脑血管取栓并发症的发生。结果 115例机械取栓患者中,发生并发症41例(35.65%)。不同部位脑血管并发症发生率分别为:MCA 44.44%(24/54),ICA31.82%(7/22),BA 22.73%(5/22),CCA 50%(1/2),VA-BA串联33.33%(2/6),ICA-MCA串联40%(2/5),前循环40.96%(34/83),后循环21.88%(7/32),不同部位脑血管机械取栓并发症发生率对比,差异无统计学意义(P>0.05)。结论前循环机械取栓并发症的发生率较高,但前后循环取栓并发症的发生无明显差异性。Objective To analyze the differences in the occurrence of complications in the treatment of cerebral vascular mechanical thrombectomy at different sites, in order to summary the experience, guiding clinical treatment. Methods The data of 115 patients with acute ischemic stroke treated with Solitaire AB stent thrombectomy in the department of neurology, Baotou Central Hospital were studied, according to the patients with thrombectomy sites, they were divided MCA,ICA,VA,BA,VA-BA,ICA-MCA, Anterior circulation and posterior circulation, the complications of cerebral embolism in each site were compared. Results In 115 cases of MT, complications occurred in 41 cases（35.65%）.The incidences of complications of different occluded vessels were,MCA 44.44%（24/54）,ICA 31.82%（7/22）,BA 22.73%（5/22）,CCA 50%（1/2）, VA-BA series 33.33%（2/6）,ICA-MCA series 40%（2/5）,Anterior circulation 40.96% （34/83）, posterior circulation 21.88% （7/32）.There was no significant difference in the incidence of complications of different occluded vessels（P〉0.05）.Conelusion The incidence of complications of anterior circulation MT was high, but there was no significant difference in the incidence of complications between anterior and posterior circulation thrombectomy.
