影像引导下立体定向脑内病变活检术21例临床分析

摘　　要：目的分析研究影响学引导下立体定向技术对于脑内病变的诊断价值,探讨手术技巧。方法使用CRW立体定向头架,采用螺旋CT薄层扫描与MRI扫描图像三维重建,对21例影像学不典型的病人行立体定向活检术。结果 21例病人均取得病理结果,诊断的成功率90.5%,其中淋巴瘤5例,星型细胞瘤9例,大脑胶质瘤病3例,生殖细胞瘤1例,脑软化1例。单发病例10例,多发病例11例。其中脑干4例,基底节3例,丘脑7例,松果体区1例,胼胝体3例,额叶2例,颞叶1例。未出现出血、偏瘫等并发症。结论脑立体定向活检术是一种可靠的临床诊断手段,定位准确,损伤小,并发症少,活检准确率高,为下一步治疗提供病理学的证据。Objective To investigate the diagnostic value of stereotactic biopsy for brain lesions under CT and MRI guidance,meanwhile ,explore the operation technique .Method Apply Cosman-Roberts-Wells□ CRW□ stereotactic head frame,thin-slice spiral CT scan and MRI scan three-dimensional reconstruction,perform stereotactic biopsies in 21 cases with nontypical CT scan and MRI scan.Pathology Results Of all the 21 cases were obtained, success rate of the diagnosis is 90.5%,in which 5 cases of lymphoma,9 cases of astrocytoma, 3 cases of brain glioma ,1 case of goniolma and 1 case of softening of the brain.There are 10 cases of single cases and 11 cases of multiple cases,in which 4 cases of brainstem, 3 cases of basal ganglia,7 cases of thalamus , 1 case of pineal region,3 cases of corpus callosum,2 cases of frontal lobe and 1 case of temporal lobe.No complications such as hemorrhage and hemiplegia appeared.Conclusion Stereotactic biopsy for brain lesions can present a reliable clinical diagnostic method with accurate location,minor injuries, few complications and high accuracy rate, it can provide pathological basis for subsequent therapies.
