摘 要:目的:探讨比较SPECT-CT与MRI在鼻咽癌早期颅底侵犯方面的检测能力。方法:选取2014年10月-2015年9月本院经病理确诊的鼻咽癌(NPC)患者60例,经患者同意后,拟治疗前2周内行鼻咽部增强MRI和SPECT-CT检查。另选取20例无全身骨转移、无颅底骨侵犯的非NPC患者作为对照参考组(了解正常头颅SPECT-CT影像表现及计算L/O值的均数±2个标准差,以确定半定量分析标准的正常值范围),比较SPECT-CT与MRI在鼻咽癌早期颅底侵犯方面的检测能力。结果:60例鼻咽癌患者,经MRI检查颅底骨侵犯阳性患者55例,阳性检出率为91.7%(55/60);经SPECT-CT检查颅底骨侵犯阳性患者54例,阳性检出率为90.0%(54/60),经SPECT-CT同机融合检查出的阳性检出率与MRI检查的阳性检出率比较,差异无统计学意义(字2=2.34,P>0.05)。结论:SPECT-CT同机融合显像对鼻咽癌颅底侵犯检测与MRI无明显差异,而SPECT-CT同机融合显像是在全身骨显像基础上进行的局部检查,没有明显增加患者的经济负担,可以在临床推广应用。Objective: To discuss and compare the detection ability of SPECT/CT and MRI in skull base invasion of nasopharyngeal carcinoma.Method: 60 patients with nasopharyngeal carcinoma ( NPC ) diagnosed by pathology from October 2014 to September 2015 in our hospital were enrolled, after the consent of the patient who were detected by enhanced SPECT-CT and MRI before 2 weeks of treatment. The other 20 cases without bone metastasis, skull base invasion of non NPC patients were enrolled as control group ( reference understanding of normal brain SPECT-CT imaging and L/O value calculation mean ±2 standard deviation, to determine the normal range of semi quantitative analysis standard ), the ability of SPECT-CT and MR1 in the detection of early skull base invasion in nasopharyngeal carcinoma was compared.Result: There were 55 cases of positive skull base bone invasion in the 60 cases of nasopharyngeal carcinoma examined by MRI, and the positive rate was 91.7% ( 55/60 ), there were 54 eases of positive skull base bone invasion examined by SPECT-CT, of which the positive rate was 90.0% ( 54/60 ), the positive rate of SPECT-CT fusion compared with the positive rate of MRI examination, there was no statistically significant difference ( χ^2=2.34, P〉0.05 ) .Conclusion: There is no significant difference between SPECT-CT fusion imaging in the detection of skull base invasion and MRI, the SPECT-CT fusion imaging is a local examination on the basis of whole body bone imaging, it can not increase the economic burden of patients, and can be widely used in clinical practice.
