摘  要:目的应用锥形束CT(CBCT)分析胰腺癌图像引导放射治疗中的摆位误差,为设计放射治疗临床靶区外放到计划靶区体积提供数据参考。方法应用Varian Trilogy加速器机载影像系统(OBI)对16例图像引导放射治疗的胰腺癌患者采集每次摆位后、摆位误差修正后及治疗后CBCT图像,并分别与计划CT图像进行匹配,获得摆位后、摆位误差修正后及治疗后在X轴(左右)、Y轴(头脚)及Z轴(腹背)3个方向的摆位误差并进行分析。结果16例患者共获取128组图像,X轴、Y轴及Z轴3个方向的摆位后的误差分别为(1.7±0.4)mm、(2.7±0.7)mm、(1.3±0.3)mm;修正后3个方向的误差均低于修正前,差异有统计学意义(P<0.05);治疗后3个方向的误差与修正后比较,差异无统计学意义(P>0.05)。结论胰腺癌图像引导放射治疗中行CBCT扫描具有重要作用,能较准确、高效地修正摆位误差,提高放射治疗精度,并为放射治疗单位准确设定计划靶体积提供了依据。Objective To analyze the positioning error of imaging guided by cone beam computed tomography (CBCT) in pancreatic cancer, and to provide data reference for the design of clinical target volume (CTV) extrapolated to planning target volume (PTV). Methods The Varian Trilogy accelerator airborne imaging system (OBI) was used to collect 16 cases of pancreatic cancer patients after image-guided radiotherapy, The images were compared with the planned CT images. The errors in the three directions of X axis (left and right), Y axis (cephalopod) and Z axis (ventral dorsum) were corrected and corrected after placement. Results There were 128 images of 16 patients, and the errors were (1.7± 0.4) mm, (2.7±0.7) mm, (1.3±0.3) mm in the three directions of X, Y and Z respectively (P〈0.05). There was no significant difference between the three directions after treatment (P〉0.05). The difference between the three directions after treatment was not significant (P〉0.05), and the difference was statistically significant (P〈0.05). Conclusion CBCT is an important method in image-guided radiotherapy of pancreatic cancer. It can correct the positioning error more accurately and efficiently, and improve the accuracy of radiotherapy. It also provides the basis for accurately setting PTV.
