摘 要:为探讨N-乙酰半胱氨酸(NAC)对脓毒血症大鼠肠损伤的影响及对炎症因子表达的影响,选取清洁级SD大鼠30只,体重250~300g,随机分为假手术组(Sham组)、脓毒症组(SEP组)和N-乙酰半胱氨酸组(NAC组),每组10只;Sham组仅开腹,暴露盲肠,NAC组和SEP组行盲肠结扎穿孔术,再分别静脉给予150mg·kg^(-1)NAC和等量生理盐水;3组均于再灌注4h处死大鼠,观察小肠组织病理学结果;ELISA检测血中肠脂肪酸结合蛋白(I-FABP)和D-乳酸浓度;ELISA法和Western blot法检测血清和小肠中TNF-α、IL-6、IL-10浓度。结果表明:光镜下Sham组未出现病理损伤,SEP组损伤重,NAC组损伤减轻;与Sham组比较,SEP组I-FABP、D-乳酸显著升高(p<0.05);与SEP组比较,NAC组I-FABP、D-乳酸显著降低(p<0.05);与Sham组比较,SEP组血清中TNF-α、IL-6、IL-10显著升高(p<0.05);与SEP组比较,NAC组TNF-α、IL-6显著降低(p<0.05),IL-10显著升高(p<0.05),小肠炎症相关因子变化趋势同血清检测。N-乙酰半胱氨酸对脓毒血症大鼠肠黏膜屏障有保护作用,其机制与抑制炎症反应有关。To investigate the effect of N-acetylcysteine(NAC) on intestinal injury in rats with sepsis and its effect on the expression of inflammatory factors, 30 SD rats of clean grade, weighing 250 to 300 g, were randomly divided into sham operation group(Sham group), sepsis group(SEP group) and NAC treatment group(NAC group), 10 rats in each group. Group Sham was only exposed to laparotomy, cecum, NAC group and SEP group underwent cecal ligation and puncture operation, then to NAC 150 mg·kg^(-1)and intravenous saline. Three groups were in 4H reperfusion rats were sacrificed to observe the intestinal pathological detection. ELISA blood intestinal fatty acid binding protein(I-FABP) and D-lactate concentration; serum and intestine was detected by ELISA and Western blot in TNF-, IL-6, alpha the concentration of-10 IL. The results showed that under light microscope group Sham had no pathological injury and SEP injury group, NAC injury group reduced compared with S group, SEP group, I-FABP D-elevated lactate(p <0.05). Compared with SEP group, NAC group I-FABP, D-lactic acid decreased(p<0.05). Compared with Sham group, SEP group serum TNF-, IL-6, IL alpha-10 increased(p<0.05). Compared with SEP group, NAC group decreased TNF-alpha and IL-6(p<0.05), IL-10 increased(p<0.05), the trend of changes of intestinal inflammation related factors with serum. N-acetylcysteine has protective effect on intestinal mucosal barrier in rats with sepsis,and its mechanism is related to inhibiting inflammatory reaction.
