顽固性与普通性复发性阿弗他溃疡体液和细胞免疫水平的比较研究

摘　　要：目的:比较顽固性与普通性复发性阿弗他溃疡(RAU)外周血体液和细胞免疫指标水平的差异。方法:选取顽固性RAU患者329例,普通性RAU患者296例以及健康对照组217例,采用免疫散射比浊法检测患者外周血血清中免疫球蛋白IgG、IgA、IgM、IgE和补体C3、C4的水平,流式细胞术检测淋巴细胞(CD19^+,CD3^+,CD4^+,CD8^+,CD4^+/CD8^+,NK)比例,并进行对比分析。结果:顽固性RAU与普通性RAU比较,免疫球蛋白IgG、IgA、IgE水平升高,补体C3、C4水平升高(P<0.05),淋巴细胞亚群中CD19^+和CD4^+/CD8^+下降,CD8^+升高(P<0.05);与对照组相比,IgG、IgA、IgE和C3、C4水平升高,CD19^+、CD4^+和CD4^+/CD8^+下降,CD8^+升高,CD3^+和NK升高(P<0.05)。普通性RAU与对照组相比,C3和C4升高,CD4^+下降(P<0.05)。结论:与普通性RAU相比较,顽固性RAU患者临床特征及免疫学指标均存在差异,其体液免疫和细胞免疫改变频次和幅度更加明显。Objective：The purpose of the study was to compare the differential levels of serum humoral and cellular immunity indexes in peripheral blood between refractory and general recurrent aphthous ulcer（ RAU ）. Methods ：Three hundred and twenty-nine patients with refractory RAU,296 patients with general RAU and 217 healthy controls were included in the study. Immunoturbidimetry was used to examine the level of immunoglobulins including IgG, lgA, IgM, IgE and those of complements such as C3 and C4 levels in peripheral blood. Flow cytometry was adopted to detect the perc＇entage of lymphocytes subsets including CD19 ＋ , CD3＋ , CIM ＋ , CD8 ＋ lymphocytes and NK cell. Results： Compared with the general RAU group,the levels of IgG,IgA and IgE increased in the refractory RAU group,so did C3 and C4 and CD8 ＋ （P 〈0. 05） ,while that of CD19＋ and CD4＋/CD8＋ decreased in the refractory RAU gaoup （P 〈 0. 05 ）. Compared with the healthy control group, the levels of IgG, IgA, IgE, C3 and C4 increased in refractory RAU group, so did that of CD8＋ and those of CD3 ＋ and NK（ P 〈 0. 05） ,while that of CD19 ＋ , CD4 ＋ and CD4 ＋/CD8 ＋ decreased in refractory RAU group （ P 〈 0. 05 ）. Compared with the healthy control group,the C3 level increased,the level of C4 increased,while that of CD4 ＋ decreased in the general RAU group（P 〈0. 05）. Conclusion： Both clinical characteristics and immunity indexes are differential between refractory and general RAU. The alteration frequency and extent of humoral and cellular immunity in patients with refractory RAU were more obvious than those with general RAU.
