摘　　要：目的研究柴胡联合龙骨牡蛎汤加减辨证治疗脑卒中后抑郁的临床效果。方法选取脑卒后抑郁患者98例,随机分为观察组(n=49)与对照组(n=49)。2组患者均给予卒中后常规处理办法,对照组应用百优解治疗,观察组则应用柴胡联合龙骨牡蛎汤加减辨证治疗。观察并记录2组患者治疗前后的抑郁状态、神经功能缺损状态以及临床疗效,并观察治疗期间的不良反应。结果 2组患者治疗后抑郁状态均得到较大程度改善(P<0.05),且治疗前后2组比较无统计学意义。神经功能缺损状态评分治疗前2组比较无统计学意义(P>0.05),治疗后神经功能缺损均得到较大程度改善(P<0.05),且以观察组改善更为显著(P<0.05)。2组患者临床疗效比较无统计学意义(P>0.05)。2组患者治疗前后肝肾功能、心电图等均未有明显改变。对照组失眠3例,经相关治疗后好转,观察组无明显不良反应。结论柴胡加龙骨牡蛎汤治疗脑卒中后抑郁的临床疗效与西药百优解基本相同,但在改善神经功能缺损方面显著优于西药百优解。Objective To study the clinical efficacy of Chaihu add Longgu Mull decoction in the treatment of depression after stroke. Methods 98 depression patients after stroke were randomly divided into observation group （n= 49） and control group （n=49）. Two groups of patients were given conventional treatment after stroke, the control group was treated by Fluxetine and the observation group was treated by Chaihu add Longgu Muli decoction. Observe and record the depressive state, neurologic impairment state, clinical efficacy and adverse effect during treatment period. Results The depression state of two group after treatment have received a major greater improvement （P 〈 0.05）, and there was no statistical significance between the two groups before and after treatment （P 〉 0.05）. Neurological deficit score was not statistically significant between the two groups before the treatment （P 〉 0.05）, The depression state of two group after treatment have received a major greater improvement （P 〈 0.05）, and the observation group improved more significantly （P 〈 0.05）. There was no statistical significance in clinical efficacy between the groups of patients （P 〉 0.05）. Liver and kidney function and ECG did not change significantly between the two groups before and after treatment. In the control group, 3 cases of insomnia were improved after the treatment, and there was no obvious adverse reaction in the observation group. Conclusion The clinical curative effect of Chaihu add Longgu Muli decoction and western medicine Fluoxetine in the treatment of post-stroke depression is basically the same, butChaihu add Longgu Mull decoction was significantly better than western medicine Fluoxetine in the improvement of neurologic impairment.
