摘　　要：目的探讨程序性死亡因子配体1（PD-L1）蛋白在结直肠癌组织中的表达及其临床意义。方法运用免疫组织化学方法，检测含90例结直肠癌组织的结直肠癌组织芯片中PD-L1的表达，并结合临床病理资料分析PD-L1的表达与患者预后的关系。结果PD-L1在肿瘤组织中阳性率为81．11％，在正常组织中为42．22％，差异有统计学意义（P〈0．01）；PD-L1的表达与肿瘤TNM分期相关（P=0．029），而与年龄、性别、肿瘤大小、病理分化程度无相关性（P〉0．05）。Kaplan-Meier生存曲线分析示，PD-L1低表达组的术后生存时间明显高于高表达组（P〈0．05）。多因素Cox比例风险模型分析表明，PD-L1表达是结直肠癌的独立预后因素之一（P〈0．05）。结论PD-L1可能成为结直肠癌靶向治疗的有效靶点，并可作为判断结直肠癌预后的独立指标。Objective To investigate the expression of programmed death ligand-1（PD-L1） in colorectal cancer and normal coloreetal tissues, and its clinical values. Methods The level of PD-L1 was detected by immunohisto- chemistry staining using a tissue microarray containing 90 eoloreetal cancer tissue samples. Meanwhile, the relationship between PD - L1 expression and patient prognosis was analyzed. Results The positive expression rate of PD - L1 was 81.11%, which was remarkably different from 42.22% in normal tissues （P 〈0. 001 ）. The expression of PD -L1 was significantly associated with TNM stages （ P = 0. 029 ） , rather than age, gender, tumor size, and pathological grade （ P 〉 0.05 ）. According to the Kaplan - Meier analysis, the survival time was markedly higher in the low PD - L1 expression group than that in the high PD - L1 expression group after operation （ P 〈 0.05 ）. The Cox regression model indicated that expression of PD - L1 was an independent prognostic factor for patients with eoloreetal cancer （ P 〈 0.05 ）. Conclusion The increased PD - L1 expression in colorectal cancer is associated with poor prognosis. PD - L1 can be a promising treatment target of coloreetal cancer and an independent molecular marker to evaluate the prognosis of patients with color- ectal cancer.
