摘 要:目的检测Beclin1、MAP1LC3B及mTOR在宫颈病变中的表达,并探讨其临床意义。方法采用免疫组织化学方法,检测宫颈病变中Beclin1、MAP1LC3B及mTOR的表达,并与正常宫颈组织对照。结果Beclin1在正常宫颈组、低度鳞状上皮内病变(low-grade squamous intraepithelial lesion,LSIL)组、高度鳞状上皮内病变(high-grade squamous intraepithelial lesion,HSIL)组和宫颈鳞癌组中的阳性表达率分别为95.00%、84.00%、62.00%及34.00%;MAP1LC3B的阳性表达率分别为90.00%、86.00%、54.00%及22.00%;mTOR阳性表达率分别为35.00%、44.00%、72.00%及86.00%,不同组中的表达差异均有统计学意义(P<0.01)。在宫颈癌患者中,Beclin1阳性表达与淋巴转移及临床分期相关,MAP1LC3B阳性表达与肿瘤分化相关,mTOR的阳性表达则与淋巴转移相关。MAP1LC3B与Beclin1的表达呈正相关而与mTOR的表达呈负相关。结论自噬相关基因所反应的自噬水平随着宫颈病变的进展而下降,推测自噬功能缺失可能是促使宫颈病变持续发展的原因之一。Objective To detect the expression of Beclinl, MAP1LC3B and roTOR in cervical lesions and explore their clinical significance. Methods The protein expression of Beclinl, MAP1LC3B and roTOR were evaluated by immunohistochemistry in cervical lesions. In addition, the expression was com- pared with normal cervical tissue. Results The positive expression rates of Beclinl in normal cervical group, LSIL group, HSIL group and cervical squamous cell carcinoma group were 95.00%, 84.00%, 62.00% and 34.00%, respectively. The positive expression rates of MAP1LC3B in these groups were 90.00%, 86.00%, 54.00% and 22.00%. The positive expression rates of mTOR were 35.00%, 44.005, 72.005 and 86.00%. The expression differences of three kinds of proteins in different groups were statistically significant (P〈0.01). In cervical squamous cell carcinoma group, the expression of Beclinl was correlated with clinical stage and lymphatic metastasis, MAPILC3B with differentiation and roTOR with lymphatic metastasis. The expression of LC3 was positively correlated with Beclinl and nega- tively with mTOR in cervical squamous cell carcinoma. Conclusion The level of autophagy, reflected by the key genes of autophagy, decreases with the progress of cervical lesions, and it is speculated that thedefect of autophagy may be one of the reasons for the continuous development of cervical lesions.
