血清学组合检测多种肿瘤相关抗原在食管鳞癌诊断中的意义

摘  要:目的探讨血清学组合检测多种肿瘤相关抗原(TAA)在食管鳞癌(ESCC)诊断中的意义。方法应用酶联免疫吸附法(ELISA)检测96例ESCC患者和63例健康对照者血清中4种TAA(CEA、Cyfra21-1、SCCA、NSE)的表达水平,分析组合检测这些血清TAA对ESCC诊断的特异性和灵敏性。结果 CEA、Cyfra21-1、SCCA、NSE在ESCC患者血清中的表达均高于健康对照组,差异有统计学意义(P<0.05)。单个检测ESCC患者血清中TAA阳性率普遍偏低,阳性率分别为10.4%(CEA)、17.7%(Cyfra21-1)、14.6%(SCCA)及25.0%(NSE)。对以上4种TAA进行逐步组合检测,发现血清学CEA、Cyfra21-1、SCCA、NSE联合诊断ESCC的敏感度可达44.8%,特异度能达72.4%。约登指数从1种TAA的0.066~0.309提高到4种TAA组合的0.124~0.435。结论血清学组合检测CEA、Cyfra21-1、SCCA、NSE对ESCC具有较高的诊断价值,对了解ESCC的发生、发展及转归有重要意义。Objective To evaluate the combined diagnostic value of multiple tumor-associated antigen (TAA) in esophageal squamous cell carcinoma (ESCC). Methods Expressions of four TAAs including CEA, Cyfra21-1, SCCA and NSE in sera from 96 cases of ESCC and 63 cases of normal individuals were detected by ELISA. Results The expressions of CEA, Cyfra21-1, SCCA and NSE in ESCC were all sig- nificantly higher than those in normal individuals (P〈0.05). The positive rates of tl?e four TAAs in ES- CC were generally low (10.4% (CEA), 17.7%(Cyfra21-1), 14.6% (SCCA) and 25.0% (NSE)). With the successive addition of TAA to a final total of four antigens, there was a stepwise increase of posi- tive reactions reaching to sensitivity of 44.8% and specificity of 72.4%. The Youdens index (YI) in- creased from 0. 066-0. 309 to 0. 124%. 435. Conclusion Multiple tumor markers combined detection may have higher diagnostic value for ESCC and be of great significance for genesis, development and out- come of ESCC.
