61例白血病患者血小板输注效果分析

摘　　要：目的:观察白血病患者输注血小板的效果,提高血小板的有效输注率。方法:回顾性分析61例白血病患者337次血小板输注的资料,比较输注后24h校正血小板计数增加值(CCI),判断血小板输注有效与否,综合分析患者疾病类型、年龄、性别、输注时体温情况及输血次数等因素对血小板输注效果的影响。结果:急性非淋巴细胞白血病(ANLL)患者的血小板输注有效率明显优于急性淋巴细胞白血病(ALL)患者,P<0.01;患者在体温正常的情况下(T<38.0℃)血小板输注有效率明显高于患者在发热(T>38.0℃)的情况下(P<0.05);随着患者输血次数增加,患者的输血有效率显著下降(P<0.05)。但患者的年龄差异和性别差异对血小板输注有效率未见明显影响(P>0.05)。结论:患者的疾病类型、输注时体温状况、输血次数影响患者血小板输注有效率。[Objective] To observe the effect of platelet transfusion in patients with leukemia and improve the effective infusion rate of platelets. [Method] A total of 337 patients with leukemia were analyzed retrospectively. The percentage of platelet count increased （CCI） was compared with 24 hours after infusion to determine whether platelet transfusion was effective or not. The effects of factors such as age, sex, body temperature and blood transfusion on platelet transfusion were analyzed compre- hensively. [Result] The rate of platelet transfusion was significantly higher in patients with acute nonlymphocytic leukemia （ANLL） than in acute lyrnphoblasfic leukemia （ALL） patients （ P 〈 0.01 ）. The effective rate of platelet transfusion was signif- icantly higher in patients with normal temperature （T 〈 38.0℃） than in patients with fever （T 〉 38.0℃ ） （ P 〈 0.05）. With the increase of the number of transfusion in the patients, the Wansfusion efficiency of the patients was significantly lower than that of the patients （ P 〈 0.05）. However, the difference in age and gender differences in patients had no significant effect on platelet transfection efficiency （P〉 0.05）. [Conclusion] The type of disease, the condition of infusion and the number of transfusion affect the efficiency of olatelet transfusion.
