47例脑转移癌全脑联合后程三维适形放射治疗疗效分析

摘　　要：目的观察全脑联合后程三维适形放射治疗在脑转移癌的临床疗效和毒副反应。方法选取我院2011年1月—2016年1月收治得原发灶经病理证实的脑转移癌患者47例作为研究对象,先行全脑放疗Dt30~40 Gy,1.8~2.0 GY/F,5 F/W,再针对残留病灶进行三维适形放射治疗Dt20~30 Gy,2~3 GY/F,3~5 F/W。放疗后评价疗效和毒副反应。结果全组47例中,CR 2例(4.26%),PR 32例(68.08%),SD 10例(21.28%),PD 3例(6.38%),有效率72.34%;生存质量:显效14例(29.78%),有效27例(57.45%),无效6例(12.77%),总有效率87.23%。毒副反应主要是放射性脑水肿和骨髓抑制。结论全脑联合后程三维适形放射治疗脑转移癌疗效较好,能够改善患者生活质量,且毒副反应可耐受。Objective To observe the clinical efficacy and side effects of whole brain combined with posterior course three-dimensional conformal radiotherapy in brain metastases. Methods Our hospital from January 2011 to January 2016 treated primary lesions of pathologically confirmed brain metastases in 47 cases, the first whole brain radiotherapy Dt30 - 40 Gy, 1.8 - 2.0 GY/F, 5 F/W, and then for the residual lesions for three-dimensional Conformal radiotherapy Dt20 - 30 Gy, 2 - 3 GY/F, 3 - 5 F/W. Evaluation of efficacy and side effects after radiotherapy. Results All the 47 cases, CR 2 cases （4.26%）, PR 32 eases （68.08%）, SD 10 cases （21.28%）, 3 eases of PD （6.38%）, efficiency of 72.34%; quality of life： 14 cases （29.78%）, effective in 27 cases （57.45%）, invalid in 6 cases （12.77%）, the total the efficiency of 87.23%. The main toxic side effects were radiation, brain edema and myelosuppression. Conclusion Whole brain combined with three-dimensional eonformal radiotherapy for brain metastases is better, can improve the quality of life of patients, and the toxicity can be tolerated.
