摘 要:目的探讨原发性低颅压综合征的影像学表现。方法对15例经临床证实的低颅压综合征的临床资料进行分析,患者均行头部CT平扫及头部MRI平扫+增强扫描。结果 15例患者头部CT平扫中8例未见明显异常,4例可见硬膜下积液,2例硬膜下少量血肿,1例脑膜增厚;头部MRI均可见脑下垂征象;3例垂体饱满,1例垂体增大,4例硬膜下积液,4例均呈脑膜弥漫性线样强化,1例上矢状窦迂曲扩张,2例颈椎MRI平扫均可见椎管硬膜外积液,增强均可见硬脊膜弥漫性线样强化。结论 MRI诊断原发性低颅压综合征具有特异性,CT诊断原发性低颅压综合征敏感性差。Objective To study the imaging manifestation of spontaneous intracranial hypotension syndrome. Methods The clinical data and imaging findings of 15 cases of spontaneous intracranial hypotension syndrome confirmed by clinic were retrospec- tively analyzed with literature review. 15 cases had the brain routine CT and MRI scan, in which there were 15 cases with MRI brain contrast-enhanced scan, 2 cases with spinal MRI scan. Results There was no obvious abnormal in 8 cases of head CT scan in 15 cases. Subdural effusion was found in 4 cases, small quantities of subdural hematoma in 2 cases, and dural thickening in 1 case. Head MRI all showed signs of brain sagging, pituitary fullness in 3 cases, pituitary enlargement in 1 case, subdural effusion in 4 cases, all of the 15 cases presented with brain contrast-enhanced. MRI scan appeared linear no-nodular and diffusely menin- geal enhancement; In 2 cases for spinal MRI scan, there were 2 cases with extradural fluid conlleetion, 2 cases with linear spinal dural enhancement. Conclusion MRI is specific for the diagnosis of spontaneous intracranial hypotension syndrome, CT is sensi- tivity difference for the spontaneous intracranial hypotension syndrome.
