摘  要:目的:利用磁共振波谱(MRS),分析卒中后抑郁(PSD)患者的脑代谢物变化的特点。方法:对10例急性脑梗死合并抑郁患者(PSD组)和10例梗死后无抑郁患者(非PSD组)行头颅1H-MRS检查,测量感兴趣区(ROI)双侧颞叶、丘脑、脑卒中部位及对侧正常区域的氮-乙酰天冬氨酸(NAA)、含胆碱化合物(Cho)、肌醇(mI)、肌酸(Cr)的含量,计算NAA/Cr、Cho/Cr以及mI/Cr比值。结果:PSD组双侧颞叶NAA、NAA/Cr,右侧丘脑NAA的值<非PSD组,而双侧颞叶Cho、Cho/Cr,右侧丘脑mI/Cr的值>非PSD组(均P<0.05)。结论:MRS可以反映PSD患者脑内各代谢物水平的改变,可能为PSD早期发现及发病机制等研究提供依据。Objective: To investigate the characteristics of cerebral metabolism in patients with post-stroke de- pression (PSD) using magnetic resonance spectroscopy (MRS). Methods: ^1H-MRS brain scans were taken on 10 acute stroke patients with depression (PSD group) and 10 acute stroke patients without depression (n0-PSD group). The neurochemical abnormalities including the levels of N-acetyl aspartate (NAA), Choline (Cho), Myo-inositol (mI) and Creatine (Cr) were measured and the ratios ofNAA/Cr, Cho/Cr, ml/Cr were calculated in both the temporal lobe, thalamus and the lesion site. Results: The patients with PSD had lower NAA and NAA/ Cr ratios, higher Cho and Cho/Cr ratios in the bilateral temporal lobes as well as lower NAA, higher mI/Cr in the left thalamus, compared to those in the no-PSD group (P〈0.05). Conclusion: The characteristics of cerebral me- tabolism in PSD can be determined by MRS, which may be helpful for early diagnosis and understanding the pathogenesis of PSD.
