乌司他丁对围术期腹腔热灌注化疗患者脑功能保护和抗炎作用的研究

摘　　要：目的研究乌司他丁对行腹腔热灌注化疗的手术患者在围术期的脑功能保护和抗炎作用,以了解乌司他丁在腹腔热灌注化疗手术患者中的应用价值。方法 56例腹腔热灌注化疗手术患者采用随机数字法分为两组(n=28):对照组(C)进行常规围术期治疗;乌司他丁组(Uli)在常规围术期治疗的基础上加用乌司他丁治疗,将两组患者在治疗前和治疗后的1d、3d、5d时的脑氧代谢参数、血清神经功能指标以及血清炎性相关因子进行统计与比较。结果与术前时比较,Uli及C组在术后1d、3d与5d时SjvO_2、Da-jvO_2和CERO_2降低(P<0.05);治疗后1 d、3 d与5 d Uli组的S-100β均明显低于同时间点C组的水平(P<0.05);治疗后1 d、3 d与5 d时Uli组患者的IL-6、TNF-α、IL-4、IL-10和IL-13均明显低于同时间点C组(P<0.05)。结论乌司他丁对行腹腔热灌注化疗的手术患者在围术期具有良好的脑功能保护和抗炎作用。Objective To observe the influence of ulinastatin for the cerebral protection and inflammatory reaction of patients underwent ontinuous hyperthermic peritoneal permsion che- mother （HIPEC） surgery during the perioperative period,in order to understand the application value of ulinastatin in the patients underwent HIPEC. Methods 56 patients were randomized into 2 groups （n =28 each） using a random number table：control group （group C） and ulinastatin group （group Uli）. 28 patients in control group were treated with conventional perioperative treatment,28 patients in Uli group were treated with ulinastatin on the conventional periopera- tive treatment,then the arteriovenous blood 02 difference and cerebral 02 extraction rate, serum nerve function related indexes, pro -inflammatory cytokines and anti -inflammatory cytokines of two groups before the treatment and on 1st,3rd and 5th day after the treatment were analyzed and compared. Results There is no significant difference in serum nerve function related indexes, pro -inflammatory cytokines and anti -inflammatory cytokines between the two groups（P 〉0.05）,while those statistical indexes of observation group on 1st, 3rd and 5th day after the treatment were all significant better than those of control group （P 〈 0.05）. The parameters of cerebral oxygen metabolism were all within the normal range in both groups. Conclusion Ulinastatin shows good effects on cerebral protection and inflammatory reaction of patients underwent HIPEC surgery during the perioperative period.
