血液AT-Ⅲ和ALB在急性脑梗死病变中的意义

摘　　要：目的:探讨血液中抗凝血酶Ⅲ(AT-Ⅲ)活性与白蛋白(ALB)含量在急性脑梗死(ACI)中的变化和临床意义。方法:选取60名新被确诊的急性脑梗死患者作为观察组,同时选择60名健康体检人员作为对照组,检测两组血浆AT-Ⅲ活性和血清ALB含量,分析观察组AT-Ⅲ和ALB之间的相关性,采用受试者工作特征曲线(ROC)评价AT-Ⅲ活性和ALB含量对ACI的诊断性能。结果:与对照组相比,观察组患者血浆AT-Ⅲ活性和ALB含量明显降低(P<0.05),两者无相关性(P>0.05);AT-Ⅲ诊断ACI临界值为91.5%,ROC曲线下面积为0.754(95%CI为0.647~0.860);ALB诊断ACI临界值为39.5g/L,ROC曲线下面积为0.759(95%CI为0.655~0.864)。结论:ACI患者AT-Ⅲ活性和ALB含量明显降低,对ACI具有良好的诊断意义。Objective ：To investigate clinical significance of plasma antithrombin（AT-Ⅲ） level and albumin （ALB）in patients with acute cerebral infarction. Methods：60 patients who were suffering from ACI were selected as ACI group, and 60 healthy subjects were served as control group. The plasma AT- Ⅲ activity and the serum ALB of two groups were detected. The correlation between AT- Ⅲ activity and ALB was evaluated in ACI group. Receiver operating characteristic curve （ROC） was used to analyze the diagnostic performance of AT- Ⅲ activity and the serum ALB for ACI. Results： Compared with control group, AT-Ⅲ activity and ALB in ACI group were lower （P〈0.05）; There was no significant correlation between AT- Ⅲ activity and ALB in ACI group （P〉0.05） ; When the optimum cut-offofAT- Ⅲ activity for ACI diagnosis was 91.5%, the area under ROC curve=0.754 （95% CI：0.647-0.860）;When the optimum cut-off of ALB for ACI diagnosis was 39.5g/L,the area under ROC curve=0.759（95% C1：0.655-0.864）. Conclusions：AT-Ⅲ activity and ALB decrease obviously in ACI patients and they are with good diagnostic performance for ACI.
