摘 要:[目的]探讨血浆p16基因甲基化联合p53抗体水平的检测对非小细胞肺癌早期诊断价值和临床意义。[方法]选择98例非小细胞肺癌患者为实验组,以60名健康者作对照,分别采用巢式甲基化特异性聚合酶链反应法检测其血浆p16基因甲基化,用酶联免疫吸附法(ELISA)检测其血浆p53抗体。[结果]非小细胞肺癌组的p16基因甲基化率(χ2=45.709,P<0.001)、p53抗体表达量(Z=-5.786,P<0.001)、p53抗体定性阳性率(χ2=41.638,P<0.001)均高于健康对照组。p16基因甲基化和p53抗体表达及定性检测对非小细胞肺癌预测诊断的敏感度分别为70.41%和58.16%,特异性分别为85.00%和93.33%,正确指数为0.554和0.515。p16基因甲基化和p53抗体定量联合检测结果对非小细胞肺癌诊断的敏感度为82.56%,特异性为78.33%,正确指数为0.610。p16基因甲基化与p53抗体检测具有相关性(r=0.215,P=0.029)。[结论 ]非小细胞肺癌患者血浆中p16基因甲基化与p53抗体可以作为非小细胞肺癌预测诊断的肿瘤标志物,联合检测的预测敏感度高于单独检测。[Objective] To evaluate the diagnostic value and clinical significance of p16 gene methylation and p53 gene in plasma of non-small cell lung cancer(NSCLC) patients.[Methods]p16 gene methylation in plasma of 98 NSCLC and 60 normal controls was tested by methylation-specific PCR(MSP).All the p53 antibody levels in plasma were detected with Elisa kit.[Results] The susceptibilities of p16 methylation and p53 antibody for diagnosis NSCLC were70.41% and 58.16%,respectively,while the specificities of p16 methylation and p53 antibody were 85.00% and 93.33%,respectively.The accurate indications of p16 methylation and p53 antibody were 0.554 and 0.515.The plasma distribution of p16 gene methylation and p53 antibody in NSCLC patients and health controls were statistically significant(χ^2=45.709,P〈0.01)(χ^2=41.638,P〈0.001).The susceptibility and specificity of combined detection of p16 methylation and p53 antibody were 82.56% and 78.33%,respectively,with the accurate tration of 0.610.The methylation of p16 gene was correlated with p53 antibody(r=0.215,P =0.029).[Conclusion] Methylation of p16 gene and p53 antibody could be acted as tumor marker in the diagnosis of non-small cell lung cancer.The sensitivity of joint detection is higher than that of single detection.
