摘　　要：目的总结评价行骨结合种植体种植修复15年的临床效果。方法回顾性分析行骨结合种植体种植修复2 302例患者临床资料,所有种植体共4 536枚,其中ITI种植体1 395枚,Ankylos种植体1 234枚,Bicon种植体165枚,Nobel种植体612枚,Camlog种植体122枚,BLB种植体765枚,Xive种植体176枚,Frialit-Ⅱ种植体67枚。所有患者的种植修复体上部结构均在2015年12月前完成。观察时间为3~180个月,中位随访时间为48个月。观察方法为影像学检查和临床检查,存留率采用Wheeler存留标准评估,纳入Kaplan-Meier存留曲线统计。结果共2 302例患者,4 536枚种植体,其中随访1 660例,失访642例,失访率为27.89%,失访种植体数为1 405枚,种植体失访率为30.97%。随访中有112例患者共152枚种植体确诊为种植体周围炎。有明确记录的共56枚种植体脱落。15年随访累计修复体松动35件,修复体折断6件,种植体折断1件,烤瓷冠崩瓷62件。据Wheeler存留标准统计种植体15年存留率为96.00%。满意度问卷调查结果显示,种植修复满意率为95.36%。结论研究种植体的15年存留率达到96.00%,种植修复远期效果好,存留价值高,满意率高。导致种植修复失败的主要原因为种植体周围炎。Objective To summarize and evaluate the clinical results of osseointegration implant treatment of the past 15 years. Methods A retrospective analysis was performed on 2302 patients with 4536 dental implants from Jan 2001 to Dec 2015 in the Center of Prosthodontics and Oral Implantology, Foshan Hospital of Stomatology. Among them, 1395 were ITI implants, 1234 were Ankylos implants, 165 were Bicon implants, 612 were Nobel implants, 122 were Cam- log implants, 765 were BLB implants, 176 were Xive implants, and 67 were Frialit - II implants. The median follow - up time was 48 months （3 to 180 months）. Clinical and imaging examinations were conducted, and evaluated with Wheeler＇ s survival criteria. Results A total of 2302 patients, with 4536 implants, were enrolled for screening, among whom 1660 had finished follow - up and 642 lost follow - up （27.89%） , with 1405 implants lost follow - up （30. 97% ）. Peri - im- plantitis was observed in 152 implants of 112 cases. Prostheses loose was observed in 35 eases and fracture in 6 cases. Ceramic crack was observed in 62 eases. The documented implants lost were 56. The implant survival rate was 96. 00% according to Wheeler＇s survival criteria. The questionnaire shows that satisfaction rate was 95.36%. Conclusion The implant survival rate is satisfied. Implant prostheses are with high satisfaction rate and excellent clinical efficacy with good long term prognosis. The major reason inducing implant failure is peri -implantitis.
