摘  要:目的探讨高频重复经颅磁刺激(HF rTMS)治疗抑郁症的短期增效作用及对认知功能的影响。方法 33例抑郁症患者在原有抗抑郁剂基础上接受2周10次rTMS,治疗前后采用汉密尔顿抑郁量表(HAMD-17)评定疗效,采用霍普金斯词语学习测试(HVLT-R)、范畴流畅性动物命名(SVFA)、韦氏记忆量表数字广度得分(WMS-DSS)评定认知功能。结果治疗后2周的HAMD-17总分、HVLT-R、SVFA、WMS-DSS与治疗前相比,均有显著性差异(P<0.05)。治疗后首发组HAMD总分明显下降,HVLT-R1、HVLT-R2、粗分明显提高(P<0.05);复发组与治疗前相比,所有HVLT-R得分、SVFA均显著提高(P<0.01),WMS-DSS也明显升高(P<0.05)。结论 HF rTMS对抑郁症有显著增效作用,并可不同程度改善患者的认知功能。Objective To explore the impact of high-frequency repetitive transcranial magnetic stimulation (HF rTMS) on short-term synergistic effect and cognitive fimction in depression. Methods 33 depression patients received rTMS for 10 times in two weeks on the basis of unchanged antipsychotic drugs. Before and after treatment, the HAMD-17 was used to evaluate the efficacy, and HVLT-R, SVFA and WMS-DSS were used to assess cognitive function. Results Compared with pre-treatment, statistical differences were found on HAMD, HVLT-R, SVFA and WMS-DSS after treatment for 2 weeks (P〈0.05). After treatment, the total score of HAMD of first-episode decreased significantly, and the HVLT-R1 score, HVLT-R2 score and raw score increased significantly (P 〈0.05). After treatment, the scores of HVLT-R, SVFA score and WMS-DSS score of recurrence group increased significantly (P〈0.01 or P〈0.05). Conclusions HF rTMS has synergistic effect on depression and can improve the cognitive function of patients.
