摘　　要：目的:对比研究3种不同根管预备系统在弯曲根管预备中的应用。方法:选自2012年1月至2015年1月103例有弯曲根管的牙髓炎或根尖周炎的患者患牙120颗,随机分为3组:手用不锈钢K锉组、PR OTAPER组、Mtwo组,评价根管预备和填充的效果。结果:手动不锈钢K锉组、PROTAPER组、Mtwo组治疗成功率分别为92.31%、98.44%、98.50%。手动不锈钢K锉组操作时间为16.25±3.26min,PROTAPER组与Mtwo组的操作时间分别为7.22±2.11min、7.08±3.38min,显著短于手动不锈钢K锉组(P<0.05)。手动不锈钢K锉组的并发症发生率为21.54%;PROTAPER组与Mtwo组的并发症发生率分别为5.47%、3.01%,显著低于手动不锈钢K锉组(P<0.05)。结论:Mtwo和PROTAPER机用Ni Ti旋转系统对于弯曲根管的预备效果良好,二者之间无显著性差异,发生根管预备并发症的可能性明显低于手动不锈钢K锉。Objective： To compare the application of 3 kinds of root canal preparation system in stainless steel hand K files, the PROTAPER and Mtwo nickel-titanium （NiTi） system. Methods： 120 curved teeth were selected and divided into three groups randomly. The degree of root canal curvature was determined on the initial radiograph by using Schneider＇s method. Group 1 teeth were instrumented with stainless steel hand K files, group 2 teeth were instrumented with PROTAPER files, and group 3 teeth were insmmaented with Mtwo files. The effect of root canal preparation and filling in three different systems were evaluated. Results： There was no significant difference of working time, instruments fracturing, root canal transportation, ledge and lateral penetration between PROTAPER and Mtwo NiTi rotary root canal preparation systems. There was a high risk of curved root canal preparation with stainless steel hand K files. Conclusions： Mtwo and PROTAPER machine with NiTi rotation system for curved root canal preparation effect is good and there is no significant difference between the two groups. They had more advantages than conventional hand files during preparation curved root canals.
