摘 要:目的:探讨中老年急性髓细胞白血病患者与牙周炎的相关性。方法:共选取102例研究对象:AML组49例,对照组53例,每组按年龄又分中年组和老年组。所有研究对象均使用Flor ida探针进行全口牙周检查。记录探诊深度(PD)、临床附着丧失(CAL)、牙龈出血指数(BI)、菌斑指数(PLI)、余留牙数、中性粒细胞(NEUT)、血小板(PLT)、C-反应蛋白(CR P),进行分析比较。结果:中年组中,AML组的PD、CAL、BI、PLI、CR P及牙周炎检出率明显高于对照组,NEUT、PLT均明显低于对照组(P<0.05);两组余留牙数之间的差异无统计学意义(P>0.05)。老年组中,AML组的PD、CAL、BI、CRP明显高于对照组,NEUT、PLT均明显低于对照组(P<0.05),而两组的PLI、余留牙数及牙周炎检出率之间的差异无统计学意义(P>0.05)。AML组牙周临床指标与相关血液指标之间不具有相关性(P>0.05)。结论:中老年急性髓细胞白血病与牙周炎关系密切,急性髓细胞白血病患者的牙周状况更差。Objective: To investigate the relationship between acute myelocytic leukemia and periodontitis in middle-aged and aged adult. Methods: A total of 102 cases were selected. 49 patients with acute myelocytic leukemia (AML group) and 53 age-matching healthy individuals(control group). Each group was divided into the middle-aged group and the aged group according to age. All subjects were examined using the Florida probe. To compare probing depth (PD), clinical attachment loss (CAL), bleeding index (BI), plaque index (PLI), number of teeth, neutrophil (NEUT), blood platelet (PLT) and C-reactive protein (CRP) were analyzed and compared. Results: In the middle-aged group, the PD, CAL, BI, PLI, CRP and the rate of periodontitis of AML group were significantly higher than those of control group, and NEUT, PLT of AML group were significantly lower than those of control group (P〈0.05). There was no significant difference in number of teeth (P〉0.05). In the aged group, the PD, CAL, BI, CRP of AML group were significantly higher than those of control group, and NEUT, PLT of AML group were significantly lower than those of control group (P〈 0.05). There was no significant difference in PLI, the number of teeth and the rate ofperiodontitis(P〉0.05). Statistically significant association between periodontal and hematological parameters in AML group was not found (P〉0.05). Conclusion: Acute myelocytic leukemia was closely related with periodontitis in middle-aged and aged adult. The patients with AML had worse periodontal status compared to healthy subjects of control group.
