全胸腔镜与后外侧开胸手术对非小细胞肺癌患者创伤指标、免疫功能及生活质量的影响

摘　　要：目的探讨全胸腔镜与后外侧开胸手术对非小细胞肺癌患者创伤指标、免疫功能及生活质量的影响。方法选取我院2014年5月至2016年5月收治的非小细胞肺癌患者80例,依据患者意愿将分为全胸腔镜手术组(n=40)和后外侧开胸手术组(n=40)两组,对两组患者的创伤指标、免疫功能及生活质量进行统计分析。结果全胸腔镜手术组患者的血清CRP、IL-6、IL-2R、SAA水平均显著低于后外侧开胸手术组(P<0.05),CD_4^+、CD_8^+、CD_4^+/CD_8^+均显著高于后外侧开胸手术组(P<0.05),PF、RF、EF、CF、SF、GOL评分均显著高于后外侧开胸手术组(P<0.05),FA、NV、PA、DY、SL、AL、CO、DI、FD评分均显著低于后外侧开胸手术组(P<0.05)。结论全胸腔镜手术较后外侧开胸手术更能有效改善非小细胞肺癌患者的创伤指标、免疫功能及生活质量。Objective To investigate the influence of thoracoscopic and posterior lateral thoracotomy on inju-ry indexes, immune function and quality of life of patients with non small cell lung cancer. Methods 80 patients with non-small cell lung cancer in our hospital from May 2014 to May 2016 were selected, and they were divided into the thoracoscopic group （n = 40） and the posterior lateral thoracotomy group （n = 40） according to their willingness. The injury indexes, immune function and quality of life of the two groups were statistically analyzed. Results The serum levels of CRP, IL-6, IL-2R and SAA in the thoracoscopic group were significantly lower than those in the pos-terior lateral thoracotomy group （P 〈0. 05） , and the value of CD4^＋ , CD8^＋ and CD4^＋/CD8^＋ were significantly higher in the thoracoscopic group than in the posterior lateral thoracotomy group （P 〈0. 05） . The scores of PF, RF, EF, CF, SF,and GOL were significantly higher in the thoracoscopic group than in the posterior lateral thoracotomy group （P 〈0. 05） , and the scores of FA, NV, PA, DY, SL, AL, CO, DI and FD were significantly lower in the thoracoscopic group than in the posterior lateral thoracotomy group （ P 〈 0. 05 ） . Conclusion Total thoracoscopic surgery is more effective in improving the injury indexes, immune function and quality of life in treatment of patients with non small cell lung cancer than posterior lateral thoracotomy group does.
