摘 要:目的:探讨DNA倍体联合肿瘤标志物检验恶性胸腔积液的临床价值。方法:70例胸腔积液患者,分为恶性组、良性组。采用全自动细胞分析仪对胸腔积液进行DNA倍体分析,并同时检测NSE、CYFRA21-1、CEA、CA199、SF。结果:恶性胸腔积液中DNA倍体和各肿瘤标志物(NSE、CYFRA2 1-1、CEA、CA199、SF)的灵敏度分别为80.00%、45.83%、76.67%、60.00%、26.67%、72.00%,特异性分别为82.50%、88.24%、70.97%、95.00%、100.00%、55.17%,准确性分别为82.43%、70.68%、73.77%、80.00%、68.11%、63.64%。组合模型中以DNA倍体串联CEA,DNA倍体并联铁蛋白诊断价值较高。结论:使用DNA细胞全自动检测分析仪对恶性胸腔积液进行DNA倍体测量具有很高的阳性率、灵敏度,如果联合胸腔积液肿瘤标志物中的CEA或SF可提高其临床诊断价值。Objective:To investigate the clinical value of DNA ploidy analysis, NSE, CYFRA21 - 1, CEA, CA199 and SF. Methods : Seventy patients with pleural fluid were divdided into malignant and benign group. All patients weredetected by quantitative DNA analysis with automated DNA image cytometry and examined for the express level of NSE,CYFRA21 -1 ,CEA,CA199,SF by enzyme - linked immunosorbent assay at the same time. Results: In malig-nant pleural fluid, the sensitivity of DNA ploidy analysis, NSE, CYFRA21 - 1, CEA, CA199 and SF were 80. 00% , 45. 83% ,76. 67% ,60. 00% ,26. 67% ,72. 00%. The specificity of DNA ploidy analysis,NSE, CYFRA21 - 1, CEA, CA199 and SF were 82.50% ,88.24% ,70.97% ,95.00% ,100.00% ,55. 17%. The accuracy of DNA ploidy analy-sis, NSE, CYFRA21 -1 ,CEA,CA199 and SF were 82.43% ,70.68% ,73.77% ,80.00% ,68. 11% ,63 .64% . In the combined models,the DNA analysis connected the SF detection in parallel and the DNA analysis connected the CEA in series had higher diagnostic value. Conclusion : DNA ploidy analysis with automated DNA image cytometry has good clinical value to diagnosis of malignant pleural fluid, especially when combined with CEA or SF.
