复方苦参注射液辅助治疗急性髓系白血病临床观察

摘　　要：目的:探讨复方苦参注射液辅助治疗急性髓系白血病的疗效,评估其对化疗药物的减毒作用,初步探索其对细胞免疫功能的影响。方法:选择初诊急性髓系白血病患者56例,随机分为两组,实验组(32例)及对照组(24例)。两组均给予同方案诱导化疗,实验组在对照组基础上加用复方苦参注射液治疗。观察粒细胞缺乏及严重血小板降低持续时间,肝功能及患者恶心、呕吐、腹泻情况;在诱导疗程结束后复查血常规、骨髓及治疗前后的细胞免疫功能。结果:联合复方苦参注射液的实验组和对照组相比,患者恶心、呕吐情况发生较少,肝功受损明显降低,粒细胞缺乏及严重血小板降低持续时间缩短(P<0.05);疗效方面两组没有显著性差异;淋巴细胞亚群在治疗后对照组均有不同程度下降(P<0.05),而实验组变化不明显(P>0.05)。结论:复方苦参注射液对于急性髓系白血病化疗疗效改善不明显,但可显著改善化疗药物毒副反应,对化疗药物有减毒作用,对于白血病患者细胞免疫功能地提升起到一定作用。Objective: To explore the auxiliary therapeutic effect of compound Kushen injection on the acute myeloid leukemia,and assess its attenuated role of chemotherapy drugs,and preliminarily explore its effect on cellular immune function. Methods: 56 patients of first diagnosis of acute myeloid leukemia,were randomly divided into two groups: The experimental group( 32 cases) and control group( 24 cases). The two groups were given with the scheme of conventional induction chemotherapy,and the experimental group was treated by compound Kushen injection on the basis of conventional chemotherapy. The duration of decreased white blood cells and serious decreased platelets,liver function and patients' nausea,vomiting and diarrhea were observed. At the end of the induction period of treatment,we tested blood routine,bone marrow and the cellular immune function of before and after the treatment. Results: In experimental group of combined with compound Kushen injection,compared to the control group,incidence of patients' nausea and vomiting were less,and damage of liver function was significantly reduced,and duration of agranulocytosis and severe platelets deficiency were shortened( P < 0. 05). There was no significant difference between two groups for the curative effect. Lymphocyte subsets in control group after the treatment were all decreased in different degree( P <0. 05),while the experimental group could not change( P > 0. 05). Conclusion: Compound Kushen injection's chemotherapy curative effect for acute myeloid leukemia cannot be improved,but can significantly improve adverse reaction of the chemotherapy drug,and the toxic effects of chemotherapy drugs could be significantly reduced,and also play a role of ascension for leukemia patients' cellular immune function.
