摘 要:目的探讨P53、c-erb B-2在颌面部鳞癌中的表达及在癌变发展中的作用。方法选取2010年1月至2011年2月来就诊并确诊为口腔面部鳞癌患者56例作为实验研究对象,均采取S-P免疫组化法测定患者抑癌基因P53表达情况,同时测定患者癌基因c-erb B-2表达情况,分析P53、c-erb B-2与颌面部鳞癌组织分化程度、临床分期以及术后5年生存的关系,并分析吸烟对颌面部鳞癌患者P53、c-erb B-2基因表达的影响。结果 56例颌面部鳞癌患者中,P53阳性表达32例,占57.14%,c-erb B-2阳性表达43例,占76.79%;组织分化程度越高,P53阳性表达越低,c-erb B-2阳性表达越高(P<0.05);临床分期越高,P53、c-erb B-2阳性表达率越高(P<0.05);且术后5年生存者、不吸烟者P53阳性表达率均较低(P<0.05)。结论 P53、c-erb B-2基因表达在颌面部鳞癌发生、发展中均有一定作用,其中P53抑癌基因可作为颌面部鳞癌治疗预后的评价指标,而吸烟可能会诱导P53抑癌基因突变。Objective To investigate the expression of P53 and c - erbB - 2 in the squamous cell carcinoma and their roles in carcinogene-sis. Methods Fifty - six patients with oral squamous cell carcinoma were selected from January 2010 to February 2011. The expression of P53 was measured by immunohistochemical method. The expression of P53 was detected by SP immunohistochemistry. The expression of c - erbB - 2 was ana-lyzed by immunohistochemistry. The relationship between P53 , c - erbB - 2 and the histological differentiation, clinical stage and 5 - year survival of the patients with maxillofacial squamous cell carcinoma were analyzed. Results Of the 56 cases of maxillofacial squamous cell carcinoma, 32 cases were positive for P53 , accounting for 57.14% and c - erbB -2 was positive in 43 cases (76. 79% ). The higher the degree of histological differentiation ,the lower the expression of P53 ( P 〈 0. 05). The positive expression rate of P53 and c - erbB - 2 was increased ( P 〈 0. 05). The positive ex-pression rate of P53 was lower in the survivors after 5 years and non - smokers ( P 〈0. 05). Conclusion The expression of P53 and c - erbB - 2 may play an important role in the development and progression of maxillofacial squamous cell carcinoma. P53 tumor suppressor gene can be used as an evaluation index for the prognosis of maxillofacial squamous cell carcinoma. Smoking may induce P53 tumor suppressor gene mutation.
