摘  要:目的探究细胞角蛋白-20(CK-20)mRNA对直肠癌的临床诊断价值。方法选取在医院行手术治疗的120例直肠癌患者,采集其术前外周血检测CK-20 mRNA表达量,并对手术切除组织进行病理学分析,研究影响CK-20 mRNA阳性表达的病理相关因素。结果直肠癌患者的CK-20 mRNA表达阳性率为46.67%;肿瘤分化程度、TNM分期、肿瘤浸润深度、淋巴结转移、肝脏转移是CK-20 mRNA阳性表达的影响因素。相关性分析显示,肿瘤分化程度、TNM分期、肿瘤浸润深度与CK-20 mRNA表达量呈显著正相关(P<0.05)。结论直肠癌患者外周血CK-20 mRNA是直肠癌微转移的敏感指标,肿瘤分化程度、TNM分期、肿瘤浸润深度是影响CK-20 mRNA表达水平的主要因素,CK-20 mRNA可作为直肠癌预后检测和肿瘤转移预防的重要参考指标。Objective To explore the value of cytokeratin-20 (CK-20) mRNA in the clinical diagnosis of rectal cancer. Methods A total of 120 rectal cancer patients to receive surgical treatment in our hospital were selected. Their preoperative peripheral blood was collected to detect the expression of CK-20 mRNA. Pathological analysis was conducted on surgical resection tissues to study the pathological factors affecting the positive expression of CK-20 mRNA. Results The positive expression rate of CK-20 mRNA among rectal cancer patients was 46.67%; tumor differentiation degree, TNM staging, depth of tumor invasion, lymph node metastasis and liver metastasis were the factors that affect the positive expression of CK-20 mRNA. Correlation analysis showed that tumor differentiation degree, TNM staging and depth of tumor invasion were significantly positively correlated to the expression of CKv20 mRNA (P 〈 0.05). Conclusion The CK-20 mRNA in the peripheral blood of rectal cancer patients is a sensitive index of the micrometastasis of rectal cancer; tumor differentiation degree, TNM staging and depth of tumor invasion are the major factors that affect the expression level of CK-20 mRNA, which may be used as an important reference index for the detection of rectal cancer prognosis and tumor metastasis prevention.
