益气通痹中药合四物汤治疗外伤性周围神经损伤疗效及对神经传导速度、NF-κB及TNF-α水平的影响

摘　　要：目的探讨益气通痹中药合四物汤治疗外伤性周围神经损伤及对神经传导速度、NF-κB及TNF-α水平的影响。方法将110例外伤性周围神经损伤患者随机分为2组,对照组55例给予电刺激干预和甲钴胺治疗,观察组55例在此基础上加用益气通痹中药合四物汤治疗,观察2组治疗前后中医证候积分、运动神经传导速度、感觉神经传导速度、NF-κB及TNF-α水平的变化,统计2组近期疗效。结果 2组治疗后四肢痿弱、手足麻木不仁、肌肤甲错、四肢青筋显露、舌质紫暗或有瘀点及苔薄白积分均显著低于治疗前(P均<0.05),且观察组治疗后各项积分均显著低于对照组(P均<0.05);2组治疗后运动神经传导速度和感觉神经传导速度均显著快于治疗前(P均<0.05),NF-κB和TNF-α水平均显著低于治疗前(P均<0.05),且观察组治疗后各项指标改善情况均显著优于对照组(P均<0.05);观察组近期治疗总有效率显著高于对照组(P<0.05)。结论益气通痹中药合四物汤治疗外伤性周围神经损伤可有效改善临床症状,加快感觉和运动神经传导速度,并有助于调节NF-κB和TNF-α水平。Objective It is to study the effects of Yiqi Tongbi Chinese Medicine with Siwu decoction on traumatic peripheral nerve damage and on the speed of nerve conduction, NF-κB and TNF-α. Methods 110 cases of traumatic peripheral nerve injury patients were randomly divided into 2 groups, the control group （ n = 55） was treated with electrical stimulation and Cobamamide, additionally the observation group （n = 55） was given Yiqi Tongbi Chinese medicine with Siwu decoction, the changes of scores of Chinese medical syndrome, motor nerve conduction velocity, sensory nerve conduction velocity, the levels of NF-κB and TNF-α before and after treatment were observed and the recent curative effects were evaluated in both groups. Results After treatment the scores of impotent weak limbs, limb numbness, tsquamous and dry skin, prominent limb veins, dark purple tongue with peteehiae and thin-white coating were significantly decreased in both groups（P 〈 0.05） , and the decrease of these indexes in observation group were more significantly than that in control group （ P 〈 0.05 ） ; motor nerve conduction velocity and sensory nerve conduction velocity were significantly faster while the levels of NF-κB and TNF-α were significantly lower than before treatment in both groups（P 〈 0.05 ） , and the improvements of these indexes in observation group were significantly better than that in control group （ P 〈 0.05 ）. The total effective rate in observation group was significantly higher than that in control group （P 〈 0.05）. Conclusion Yiqi Tongbi Chinese Medicine with Siwu decoction can effectively improve the clinical symptoms, speed up the sensory and motor nerve conduction velocity, and help to regulate the levels of NF-κB and TNF-α in the treatment of traumatic peripheral nerve damage.
