探讨血脂水平在女性无症状脑梗塞患者预后中的预测价值

摘　　要：目的探讨血脂水平对急性脑梗塞早期预后的影响。方法连续收录本院2014年门诊就诊的经三级医院CT检查发现的无症状脑梗塞的女性患者共89例,收集基线信息后前瞻性随访1年,探讨低密度脂蛋白水平等血脂水平与预后的关联。结果本研究中纳入研究的89例受试对象中LDL>4.14 mmol/L的患者52名,5.77%的患者主诉存在轻微症状,69.23%的患者病灶直径大小<15 mm;主要的病变部位集中在侧脑室周围和皮质下白质以及基底节。LDL≤4.14 mmol/L的患者占37例,5.41%的患者主诉存在轻微症状;,35.14%的患者病灶直径大小<15 mm。随访1年内,两组患者进展为症状性脑卒中的发生率分别为和51.35%vs7.31%(P<0.01)。多因素回归分析,LDL水平异常增高是女性进展为症状性脑卒中的独立危险因素,此外,TC异常,TG异常,HDL异常三种其他常见血脂指标超标也同时是进展为脑卒中的危险因素,初诊时有轻微症状以及病灶较大也同样会增加进展为脑卒中的风险。结论低密度脂蛋白水平异常增高会增加女性无症状脑梗塞患者短期内发展成为症状性脑卒中。Objective To discuss the effect of blood glucose level on the early prognosis of acute cerebral infarction. Methods 89 female patients with asymptomatic cerebral infarction were examined by CT examination. The baseline data were collected and followed up for one year. The relationship between LDL and other lipid levels was analyzed. Results 52 patients with LDL 〉 4. 14 mmol/ L were enrolled in this study. 5.77% of the patients complained of mild symptoms,69.23% of the patients had lesion diameter 〈 15 ram; the main lesion was concentrated in the periventricular and subcortical. LDL 〈 4. 14 L of 37 cases of patients, 5.41% of patients presented with mild symptoms; ,35.14% of the patients with lesions diameter size 〈 15 mm. white matter and basal ganglia. After one year the two groups incidence of symptomatic stroke was 51.35% ,vs7.31%. The total dyslipidemia of LDL 〉4. 14 mmol/ L group was still more seriousIn addition, TC ,TG,and HDL abnormalities were the risk factors of stroke,minor symptoms and larger lesions at the time of initial diagnosis also increase the risk of progression to stroke. Conclusion The abnormally high level of LDL may increase the development of symptomatic stroke in female patients with asymptomatic cerebral infarction in the short term.
