摘 要:目的 探讨鼠神经生长因子对食管癌放射疗法后吞咽困难患者的效果.方法 选取2012年7月至2016年3月放疗后吞咽困难的食管癌患者114例,依据随机数字表法分为观察组(鼠神经生长因子+常规治疗)和对照组(常规治疗),比较两组患者的临床疗效及吞咽功能.结果 观察组57例,其中显效22例(38.60%),有效25例(43.86%),无效10例(17.54%),总有效率为82.46%;对照组57例,其中显效8例(14.04%),有效19例(33.33%),无效21例(36.84%),恶化9例(15.79%),总有效率为47.37%,两组总有效率比较差异有统计学意义(P〈0.05).两组治疗前咽启动延迟时间、喉部运动幅度、无效吞咽次数比较差异未见统计学意义(P〉0.05).观察组治疗后咽启动延迟时间、无效吞咽次数明显低于对照组,喉部运动幅度显著高于对照组,差异有统计学意义(P〈0.05).两组治疗前洼田饮水试验分级、FOIS评分、VFSS评分比较差异均未见统计学意义(P〉0.05).观察组治疗后洼田饮水试验分级明显低于对照组,FOIS评分及VFSS评分显著高于对照组,差异有统计学意义(P〈0.05).结论 鼠神经生长因子能够明显改善食管癌行放射疗法后吞咽困难患者的吞咽功能,对修复患者受损神经具有重要意义.Objective To investigate the effect of mouse nerve growth factor on esophageal dysphagia patients after radiation therapy.Methods From July 2012 to March 2016,114 esophageal cancer patients after radiotherapy with dysphagia were selected,based on the random number table method,they were divided into observation group (mouse nerve growth factor and routine therapy) and control group (conventional treatment).The clinical efficacy and swallowing function were compared between the two groups.Results There were 57 cases in observation group,markedly effective in 22 cases (38.60%),effective in 25 cases (43.86%),invalid in 10 cases (17.54%),the total effective rate was 82.46%;in the control group (57 cases),markedly effective in 8 cases (14.04%),effective in 19 cases (33.33%),invalid in 21 cases (36.84%),deterioration in 9 cases (15.79%),the total effective rate was 47.37%,the difference of the total effective rate between the two groups was significant (P〈0.05).Before treatment in both groups,the started pharyngeal delay time,range of motion throat,swallowing invalid number had no significant difference (P〉0.05).After treatment,the pharyngeal start delay time,the number of invalid swallowing were significantly lower than those in the control group,throat range of motion was significantly higher,the differences were significant (P〈0.05).Before treatment in both groups,water drinking test grading Kubota,FOIS score,VFSS scores had no significant difference (P〉0.05).After treatment,the water drinking test grade Kubota in observation group was significantly lower than that in the control group,FOIS score and VFSS score were significantly higher,the differences were significant (P〈0.05).Conclusions Mouse nerve growth factor can significantly improve esophageal row after radiation therapy in patients with dysphagia swallowing function of patients,to repair damaged nerve is important.
