不同切口超声乳化术联合小梁切除术治疗白内障合并青光眼疗效研究

摘　　要：目的 探讨不同切口超声乳化术联合小梁切除术治疗白内障合并青光眼的临床疗效.方法 选取2014年6月至2016年5月收治的白内障合并青光眼患者96例,按照随机数字表法分成观察组和对照组各48例,观察组采用双切口超声乳化术联合小梁切除术治疗,对照组采用单切口超声乳化术联合小梁切除术治疗,观察两组治疗前后眼内压、视力情况、滤过泡形成情况和并发症发生率.结果 在眼内压上,两组治疗后与治疗前比较差异有统计学意义（P＜0.05）,观察组治疗后明显低于对照组,差异有统计学意义（P＜0.05）;在裸眼视力和最佳矫正视力上,两组治疗后与治疗前比较均有显著提高,差异有统计学意义（P＜0.05）,观察组治疗后明显高于对照组,差异有统计学意义（P＜0.05）;观察组功能型滤过泡形成率明显高于对照组,差异有统计学意义（P＜0.05）,两组在微小型、缺如型、包裹型、弥漫扁平型滤过泡形成率上比较差异无统计学意义（P＞0.05）;两组在并发症发生率上比较差异无统计学意义（P＞0.05）.结论 与单切口相比,双切口超声乳化术联合小梁切除术治疗白内障合并青光眼疗效更为显著,能够有效改善眼内压,促进视力恢复.Objective To evaluate the clinical efficacy of different incision phacoemulsification and trabeculectomy for cataract and glaucoma.Methods Ninety-six patients with cataract and glaucoma were selected from June 2014 to May 2016 in our hospital,according to randomly divided into the observation group and the control group,each of 48 cases,the observation group used double incision phacoemulsification combined with trabeculectomy resection,and the control group used single incision phacoemulsification combined with trabeculectomy resection,IOP,visual acuity,bleb formation and complication rates before and after treatment were observed.Results In IOP,both groups after treatment were significantly lower than before treatment （P 〈0.05）,the observation group after treatment was significantly lower than the control group （P 〈0.05）;in the uncorrected visual acuity and best corrected visual acuity,both groups after treatment were significantly increased than before treatment （P 〈0.05）,the observation group after treatment was significantly higher than the control group （P 〈0.05）;the functional bleb formation rate of the observation group was significantly higher than the control group （P 〈0.05）,there were no significant difference in miniature type,absence type,package type and flat diffuse type bleb formation rate of two groups （P 〉0.05）;there was no significant difference in the incidence of complications of two groups （P 〉0.05）.Conclusions Compared with single incision,double incision phacoemulsification and trabeculectomy for cataract and glaucoma has more significant effect,it can effectively improve intraocular pressure,promote visual acuity.
