摘 要:目的探讨非酒精性脂肪肝(NAFLD)的患病情况与代谢综合征(MS)相关指标变化的关系。方法收集某高校3071例教职工的体检完整资料,分析NAFLD与脂代谢异常、高血压、高血糖、高尿酸水平的关系,并研究不同年龄、性别的NAFLD患病情况。结果 3071例研究对象中,共检出NAFLD患者1022例,患病率为33.28%。随着年龄增长,发病率逐渐升高,男性NAFLD患病率高于女性,差异有统计学意义(P<0.01)。NAFLD组脂代谢异常、高血压、高血糖、高尿酸患病率明显高于非NAFLD组,差异有统计学意义(P<0.01)。NAFLD组体质量指数(BMI)、收缩压(SBP)、舒张压(DBP)、甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、空腹血糖(FBG)、谷丙转氨酶(ALT)、尿酸(UA)水平明显高于非NAFLD组,差异有统计学意义(P<0.01)。NAFLD组TG的OR=1.523,BMI的OR=1.325,FBG的OR=1.156,DBP的OR=1.429。由此可见,BMI、FBG、TG、DBP的测试值与NAFLD明显相关。结论 NAFLD的发生不仅与脂代谢障碍有关,而且与年龄、性别以及合并有高脂血症、高血压、高血糖、高尿酸等多种代谢紊乱的全身性疾病密切相关。Objective To explore the correlation between non-alcoholic fatty liver disease (NAFLD) prevalence situation and metabolic syndrome (MS) related indexes changes. Methods Complete medical information of 3071 faulty members was collected to analyze correlation between NAFLD and lipid metabolism disorders, hypertension, hyperglycemia, hypeluricemia level, and NAFLD prevalence situation was studied in different age and gender. Results Among 3071 cases, there were 1022 NAFLD cases, with prevalence rate rate as 33.28%. The incidence of disease rising with age, and male had higher NAFLD prevalence rate than female. The difference had statistical significance (P〈0.01). NAFLD group had obviously higher prevalence rate of lipid metabolism disorders, hypertension, hyperglycemia, hypeluricemia level than non NAFLD group, and the difference had statistical significance (P〈0.01). NAFLD group had obviously higher body mass index (BMI), systolic blood pressure (SBP), diastolic blood pressure (DBP), triglyceride (TG), total cholesterol (TC), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterin (LDL-C), fasting blood-glucose (FBG), alanine aminotransferase (ALT) and uric acid (UA) level than non NAFLD group, and their difference had statistical significance (P〈0.01). NAFLD group had OR value of TG as 1.523, BMI as 1.325, FBG as 1.156 and DBP as 1.429. BMI, FBG, TG and DBP test value were obviously correlated to NAFLD. Conclusion NAFLD occurrence was not only correlated to lipid metabolism disorders, but also closely correlated to age, gender and systemic disease of hyperlipidaemia, hypertension, hyperglycemia and hypeluricemia.
