曲安奈德穴位注射联合中药皮肤康洗液治疗肛门湿疹的临床研究  

Clinical Study on Acupoint Injection of Triamcinolone Acetonide Combined with TCM Pifukang Solution in Treating Anal Eczema

摘　　要：目的:探讨曲安奈德穴位注射联合中药皮肤康洗液在肛门湿疹临床治疗中的应用价值。方法:选取2013年3月—2015年12月于本院就诊的124例肛门湿疹湿热证患者为受试对象,随机数字表法分为研究组和对照组各62例。对照组中途脱落4例,共58例有效病例入组;研究组中途脱落6例,共56例有效病例入组。对照组:温水坐浴+糠酸莫米松乳膏外涂方案。研究组:曲安奈德穴位注射联合中药皮肤康洗液外洗方案。对比两组受试者临床疗效及不良反应发生率差异,记录其治疗前后相关指标[血清白细胞介素-2(IL-2)、白细胞介素-4(IL-4)、白细胞介素-6(IL-6)和胃泌素(Gas)]检测结果、皮损最大直径及症状(瘙痒、灼热、疼痛、大便秘结、小便短赤、口干心烦)积分变化情况。结果:研究组治疗总有效率显著高于对照组(91.1%vs 72.4%,P<0.05)。治疗2周后,两组受试者血清IL-2水平均较治疗前显著提升,且研究组>对照组;部分指标(血清IL-4、IL-6、Gas)检测水平、皮损最大直径检测结果及症状积分评估结果则较治疗前显著降低,且研究组<对照组,差异均有统计学意义(P<0.05)。研究组治疗后出现月经紊乱、头晕、恶心症状各1例,对照组出现局部烧灼感1例,两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:对肛门湿疹患者予以曲安奈德穴位注射联合中药皮肤康洗液坐浴治疗,能有效缓解症状,于其病情转归有利。Objective：To explore the application value of acupoint injection of triamcinolone acetonide combined with TCM Pifukang solution in the clinical treatment of anal eczema. Methods ：A total of 124 patients with anal eczema and damp -heat syn- drome treated in our hospital between March 2013 and December 2015 were randomized into the study group and the control group, with 62 cases in each. There were 4 cases lost to follow up and 58 effective cases in the control group,6 eases lost to follow up and 56 effective cases respectively in the study group. The control group was treated with warm water bath and external applica- tion of Mometasone Furoate Cream. The study group was treated with acupoint injection of triamcinolone acetonide combined with external washing with TCM Pifukang solution. The clinical efficacy and incidence of adverse reactions were compared between the two groups. The test results of related indexes [ serum interleukin - 2 （ IL - 2）, interleukin - 4 （ IL - 4）, interleukin - 6 （ IL - 6）, gastrin （Gas） ], the largest diameter of skin lesion and changes of scores of symptoms （ itching, burning, pain, constipation, scanty dark urine, dry mouth and upset） were recorded before and after treatment. Results：The total effective rate of the study group was significantly higher than that of the control group （91.1% vs 72.4% ,P 〈0. 05 ）. After 2 weeks of treatment,the lev- els of serum IL - 2 in the two groups were significantly higher than those before treatment and the levels in study group were high- er than the control group. The detection levels of some indicators （ serum IL - 4, IL - 6, Gas ） , the largest diameter of skin lesion and scores of symptoms were significantly lower than those before treatment and those in study group were lower than that of con- trol group （ P 〈 0. 05 ）.
