摘 要:目的应用经阴道超高速剪切波弹性成像测定宫颈组织弹性模量值,探讨该技术对宫颈癌及癌前病变的诊断价值。方法选取2015年5月—2016年5月在济宁医学院附属医院经手术病理检查确诊的宫颈癌患者30例(A组)、宫颈上皮内瘤变(CIN)患者50例(B组);另选取同期健康已婚女性50例(C组)为对照。3组受试者术前均进行经阴道常规二维超声、彩色多普勒及超高速剪切波弹性成像检查,并获得弹性模量最大值、平均值。结果A组弹性图像以红色或橘红色为主,夹杂着绿色或呈亮绿色;B组和C组弹性图像呈较均匀的蓝色。A组弹性模量最大值、平均值分别为(94.54±32.02)、(73.96±29.98)kPa,B组分别为(20.73±4.21)、(16.72±3.60)kPa,C组分别为(20.72±5.25)、(16.16±3.97)kPa,A组弹性模量最大值、平均值大于B、C组(P<0.05),B组和C组间差异无统计学意义(P>0.05)。结论宫颈癌病灶的弹性模量最大值、平均值高于癌前病变及正常组织,超高速剪切波弹性成像对诊断宫颈癌有重要价值。Objective To obtain the elastic modulus of cervical lesions by using ultrafast shear wave elasticity imaging(SWEI),and based on this,to investigate the value of SWEI in the diagnosis of cervical carcinoma and cervical precancerous lesions.Methods The participants enrolled in this study were all from Affiliated Hospital of Jining Medical University,including 30 diagnosed with cervical cancer(group A),50 with cervical intraepithelial neoplasia(CIN)(group B)by pathological examination of surgical specimens,and 50 healthy married women undergoing health examination and visiting the Gynecological Outpatient(group C) from May 2015 to May 2016.All women received the conventional transvaginal two-dimensional ultrasound imaging,Color Doppler imaging,and SWEI for obtaining the maximum and mean elastic modulus values of the cervix.Results The elasticity images of region of interest(ROI) of group A were red or orange mainly,mixed with green or bright green,and those of group B and C were homogeneous blue.Group A had greater maximum elastic modulus value of ROI than group B〔(94.54±32.02) kPa vs.(20.73±4.21) kPa,P〈0.05〕 and group C〔(94.54±32.02) kPa vs.(20.72±5.25) kPa,P〈0.05〕.The mean elastic modulus value of ROI in group A was greater than that in group B〔(73.69±29.98) kPa vs.(16.72±3.60) kPa,P〈0.05〕 or that in group C〔(73.96±29.98) kPa vs.(16.16±3.97) kPa,P〈0.05〕.Group B and group C did not differ significantly in the maximum and mean elastic modulus values of ROI(P〉0.05).Conclusion Ultrafast SWEI detected that the maximum and mean elastic modulus values of cervical carcinoma are greater than those of the CIN and normal cervical tissues,suggesting that it can provide valuable reference for the diagnosis of cervical carcinoma.
