摘 要:目的探索血糖变异性与大面积脑梗死患者预后的关系。方法选取2013年7月至2015年12月于我院神经内科重症监护室(NICU)治疗的大面积脑梗死患者111例,分为存活组88例和3个月死亡组23例,分析两组血糖情况,对平均血糖(GluAve)、血糖标准差(GluSD)、血糖变异系数(GluCV)、最大血糖波动幅度(LAGE)进行计算,分析血糖变异性与患者死亡的关系。结果死亡组GluSD、GluCV和LAGE均高于存活组[(4.75±0.46)vs(1.89±0.70)mmol/L,(53.66±4.82)vs(26.06±4。82)mmol/L,(19.03±3.51)vs(6.75±1.21)mmol/L,P<0.01],死亡组GluAve与存活组比较,差异无统计学意义(P>0.05)。结论血糖变异性与大面积脑梗死患者的死亡密切相关,比GluAv水平能更好的预测患者预后。Objective To explore the relationship between blood glucose variability and the prognosis of massive cerebral infarction. Methods A total of 111 hospitalized patients of massive cerebral infarction in our NICU from July 2013 to December 2015were included in this study. All the patients were divided into two groups., survival group (n= 88)and death group (dead in 3 months) (n= 23). Blood glucose was compared between the two groups. The values of GluAve, SD, GIuCV, and LAGE were calculated. The relationship between the blood glucose variability and prognosis of massive cerebral infarction were anlayzed. Results The GluSD, GluCV and LAGE were higher in death group than in survival group. [(4. 75±0. 46) vs (1.89±0.70) mmol/L, (53.66±4.82) vs (26.06±4. 82) retool/L, (19. 03±3.51) vs (6.75± 1.21) mmol/L, P〈0.01]. The difference of mean blood glucose was not statistically significant in death group and survival group, P 〉 0.05]. Conclusion Blood glucose variability is associated with death from massive cerebral infarction. And may be better than GluAve in prediction of prognosis.
