椎板成形术治疗脊柱椎管内肿瘤

摘　　要：目的探讨椎板成形术治疗脊柱椎管内肿瘤的疗效。方法应用椎板成形术治疗18例脊柱椎管内肿瘤患者,分析该术式对肿瘤的显露和切除情况,随访术后脊柱稳定性及并发症。结果椎管内外哑铃型肿瘤3例,硬膜下肿瘤10例,髓内肿瘤5例。手术行椎管扩大成形2~6(3.8±0.9)个节段。患者肿瘤均是全切除。患者均获得随访,时间24~52个月。JOA评分从术前8.2分±2.0分提高到末次随访时15.0分±1.9分,平均改善率为82.9%±8.8%。所有患者成形椎板复位固定情况良好,骨性融合情况良好,未出现移位,脊柱没有出现不稳定。结论椎板成形术对椎管内肿瘤显露良好,对脊柱的稳定性影响小,手术并发症少。Objective To investigate the efficacy of the laminoplasty for spinal tumors.Methods Clinical data of 18 cases of spinal tumors performed the laminoplasty were analyzed retrospectively.The surgical technique of lamina replantation and fixation were applied during the operation.The exposure and resection of tumors were analyzed.Spinal stability and complications were followed up.Results Three cases of dumbbell-shaped intra-and extra-spinal tumors,10 cases of spinal subdural tumors,and 5 cases of intramedullary tumors were included in our study.The segments of vertebral column involved in the operation varied from 2~6（3.8±0.9）segments.All tumors were completely resected.During 24 months to 52 months＆#39; follow-up,JOA score was increased from 8.2±2.0 preoperation to 15.0±1.9 postoperation,with an average improvement rate of 82.9%± 8.8%.The lamina replantation,fixation and bone-graft fusion were satisfactory in all the cases.The lamina replantation got no displacement,there was no spinal instability.Conclusions Laminoplasty is effective for tumor exposure and resection,with less influence on the spinal stability,less complications.
