360度视网膜切开联合剥除视网膜增殖膜治疗复杂性视网膜脱离的临床分析

摘　　要：目的探讨360度视网膜切开联合剥除视网膜增殖膜治疗复杂性视网膜脱离的临床效果,分析影响预后的术中注意事项。方法对2011年3月至2014年3月收治的31例(31眼)复杂性视网膜脱离患者临床资料作回顾性分析,均行360度视网膜切开联合视网膜增殖膜剥除、视网膜电凝及光凝、硅油填充术等治疗,观察术后视网膜复位、视力、并发症等情况,并分析影响预后的术中注意事项。结果 31例患者首次术后30例视网膜解剖完全复位,1例未复位者经再次手术后复位出院。术后出院时视力提高者26例(83.9%)。随访平均16个月,视网膜复位者29例(93.5%),随访末视力较术前提高者25例(80.6%)。结论360度视网膜切开联合剥除视网膜增殖膜是治疗复杂性视网膜脱离的一种有效方法,可以提高视网膜复位的成功率。Objective To investigate the therapeutic effect of 360 degree retinotomy combined with retinal proliferative membrane peeling in the treatment of complicated retinal detachment, and to analyze the surgical precautions influencing the prognosis. Methods A retrospective survey was performed in 31 patients（31 eyes） with complicated retinal edtachment（CRD） who underwent 360 degree retinotomy, retinal proliferative membrane peeling, retinal electrocoagulation, laser retinopexy and silicone oil injection in our hospital from March 2011 to March 2014. Retinal reattachment, visual acuity and complications were observed, and the surgical precautions influencing the prognosis were analyzed. Results Of 31 patients, retinal attachments of 30 cases were full reset after primary surgery, and 1 patient who did not reset discharged after reoperation; the visual acuity of 26 cases（83.9%） improved at discharge. After followed up for average 16 months, 29cases（93.5%） achieved retinal reattachment, and at the end of the follow-up, the visual acuity of 25 cases（80.6%） improved compared with that before operation. Conclusion 360 degree retinotomy combined with retinal proliferative membrane peeling is an effective treatment method for complicated retinal detachment and can improve the success rate of retinal reattachment.
