中晚期贲门癌经胃左动脉灌注栓塞治疗的临床效果

摘　　要：目的探讨中晚期贲门癌患者经胃左动脉灌注栓塞治疗的临床效果。方法回顾性分析我院收治的39例经胃左动脉灌注栓塞治疗的中晚期贲门癌患者的临床资料,所有患者均接受胃左动脉灌注栓塞治疗,观察患者的临床效果。结果 39例患者中,完全缓解11例(28.21%),部分缓解21例(53.85%);稳定4例(10.26%),进展3例(7.69%)。在治疗过程中,出现恶心、呕吐24例(61.54%);上腹部疼痛23例(58.97%);发热13例(33.33%),骨髓抑制16例(41.03%)。随访结果显示,半年生存率为84.62%(33/39),一年生存率为64.10%(25/39),两年生存率为38.46%(15/39),超过3年生存率为7.69%(3/39),其中1例已存活5年。结论经胃左动脉灌注栓塞的介入疗法可有效延长贲门癌患者的生存期,提高其生活质量,是中晚期贲门癌患者安全有效的治疗措施。Objective To observe the clinical effect of the left gastric arterial embolization in the treatment of advanced cardia cancer. Methods The clinical data of 39 cases of advanced cardia cancer patients treated with left gastric arterial embolization admitted in our hospital were retrospectively analyzed. All the patients were treated with left gastric arterial embolization, the clinical effect was observed. Results In 39 patients, there were 11 cases （28.21%）with complete remission, 21 cases （53.85%） with partial remission, 4 cases （10.26%） with stabilization and 3 cases （7.69%） with progress development. In the process of the treatment, there were 24 cases （61.54%） with nausea and vomiting, 23 cases （58.97%） with epigastric pain, 13 cases （33.33%） with fever and 16 cases （41.03%） with myelosuppression. The follow-up results showed that the six months survival rate was 84.62% （33/39）, 1-year survival rate was 64.10% （25/39）, 2-year survival rate was 38.64% （15/39）, more than 3-year survival rate was 7.69% （3/39）, 1 had survived for 5 years. Conclusion Left gastric arterial embolization in the treatment of advanced cardia cancer can effectively prolong the patients＇ lifetime and improve the patients＂ quality of life, which is a safe and effective treatment in patients with advanced cardia cancer.
