摘　　要：目的探讨乳腺原发鳞状细胞癌(SqCC)临床病理特征、免疫表型、诊断及鉴别诊断。方法收集5例乳腺原发鳞状细胞癌,总结该组病变临床资料,并通过苏木精-伊红(HE)染色、免疫组织化学染色观察该组病变的病理特征。结果 5例乳腺SqCC患者年龄为32~68岁,平均年龄54.8岁,均为女性。肿瘤大体境界较清楚,其中3例发生囊性变。肿瘤细胞胞质丰富,嗜酸性,合体细胞样,可见单个细胞角化。免疫组织化学除1病例雌激素受体(ER)及孕激素受体(PR)均3%阳性外,其余病例均阴性。5例病例AR、HER2、GCDFP-15、GATA3均阴性。鳞状细胞癌免疫标记CK5/6及P63均弥漫阳性。5例患者中其中1例无同侧腋窝淋巴结转移患者,2年后发生肝脏转移。结论 Sq CC是罕见乳腺特殊类型癌,诊断和鉴别诊断有赖于HE形态和免疫组织化学染色。其常有跨淋巴结远处转移的特性具有重要的意义,因其少见,并无规范化治疗方案。Objective To investigate the clinicopathological features, immune phenotype, diagnosis and differential diagnosis of primary squamous cell carcinoma of the breast （SqCC）. Methods We analyzed 5 cases of primary SqCC of breast by morphological observation, immunohistochemical staining and summary of clinical datas. Results Five cases of breast SqCC patients were all female, 32～68 years old. Tumors were well circumscribed and 3 cases had cystic change. The tumor cells are rich in cytoplasm, eosinophilic, syncytial like, and the keratosis can be seen in a single cell. Immunohistochemistry in addition to 1 case of ER and PR were 3% positive, the other cases were negative. The Immunohistochemistry stain of AR, HER2, GCDFP-15, GATA3 were all negative in the 5 cases. The squamous cell carcinoma immune markers of CK5/6 and P63 were diffuse positive. One patient in 5 cases had no ipsilateral axillary lymph node metastasis, and liver metastasis occurred two years later. Conclusion Primary squamous cell carcinoma of the breast is a rare special type of breast cancer. The diagnosis and differential diagnosis of this tumor depends on the morphology of HE and immunohistochemical staining. There is significant incidence of distant metastasis even without lymph node involvement in primary SCC of breast. Because it is rare, and there is no standardized treatment.
