PVP联合全雄激素阻断治疗晚期前列腺癌伴膀胱颈梗阻患者的临床分析

摘　　要：目的:探讨伴膀胱颈梗阻的晚期前列腺癌患者采取PVP联合全雄激素阻断治疗的效果。方法:选取2014年3月-2015年3月本院接收的晚期前列腺癌患者70例,按照随机数字表法分为对照组(给予经尿道前列腺电切术联合全雄激素阻断治疗,n=35)与观察组(给予PVP联合全雄激素阻断治疗,n=35),观察相关指标变化及术后情况。结果:观察组手术时间、术后留置导尿管时间、出血量、住院天数均显著低于对照组(P<0.05);两组排尿困难症状明显减轻,与术前比较差异均有统计学意义(P<0.05),且术后前列腺体积较术前明显下降(P<0.05),组间比较差异均无统计学意义(P>0.05);与术前比较,两组术后IPSS、Qmax、QOL、PVR及t PSA均有明显改善(P<0.05),而观察组QOL显著低于对照组(P<0.05),两组其余指标比较差异均无统计学意义(P>0.05);观察组并发症发生率较对照组显著要低(P<0.05)。结论:伴膀胱颈梗阻的晚期前列腺癌患者采取PVP联合全雄激素阻断治疗,可明显缓解患者症状,抑制肿瘤进展,且PVP手术具有手术时间短、并发症少、术后恢复快等优点,具有推广价值。Objective： To investigate the efficacy of PVP combined with total Androgen blockade in the treatment of advanced prostate cancer with bladder neck obstruction.Method： A total of 70 cases of advanced prostate cancer patients admitted to our hospital from March 2014 to March 2015 were randomly divided into the control group （ TURP combined with total Androgen blockade, n=35 ） and the observation group （ PVP combined with total Androgen blockade, n=35 ）, the changes of the related indexes and postoperative situation of two groups were observed.Result： The operation time, postoperative indwelling catheter time, blood loss, and hospital stay of the observation group were significantly lower than those of the control group （ P〈0.05 ） .Dysuria symptoms of two groups were significantly reduced and had significant difference compared with those of before treatment （ P〈0.05 ） .Besides, the volume of prostate volume was significantly decreased compared with that before operation （ P〈0.05 ）, and there was no significant difference between two groups （ P〉0.05 ） .The postoperative IPSS, Qmax, QOL, PVR and tPSA of two groups were significantly different compared with those before treatment （ P〈0.05 ）, while QOL of the observation group was significantly lower than that of the control group （ P〈0.05 ） .There was no significant difference for other indicators of two groups.The incidence of operative complications of the observation group was significantly lower than that of the control group （ P〈0.05 ）.Conclusion ： PVP combined with total Androgen blockade in the treatment advanced prostate cancer with bladder neck obstruction can significantly relieve the symptoms, inhibit tumor progression.Besides, PVP operation has the advantages of operation time shortening, fewer complications, and faster postoperative recovery, etc.
