全口义齿下颌前伸位的处理

摘　　要：目的探讨全口义齿下颌前伸位的处理方法。方法回顾性分析近5年来因水平颌位关系记录正中关系错误出现下颌前伸位引起全口义齿修复失败的10例患者的临床资料。用3M精细硅橡胶枪式混和液体，只需磨掉下颌左右4个后牙，重新排4个后牙，减少患者就诊的次数和迫切需要一付假牙解决咀嚼食物的困难。结果将重新制作的下颌义齿在口内试戴，咬颌关系、固位、边缘伸展良好，下领对上颌的位置、上下中线对位一致，开阁口运动下颌未见前伸，后牙牙尖尖窝相对交镨咬颌。X射线检查显示关节形态无异常，髁状突于关节凹居中。结论保留上颌义齿、修改下颌左右4个人工后牙能很好地恢复无牙颌美观、咀嚼和发音等功能。Objective To explore the process of lower jaw and take teeth＇s movementa, lvlemoas fake teeth＇s recovery failure because of lower jaw relationship make wrong record of the middle relationship caussed lower jaw move forward during the past 5 years were retrospectively analyzed. With the help of 3 m mixed liquid of silicone rubber gun type, we grind four teeth which are near lower jaw. It can reduce the times to see a doctor and relieve the urgent need for a pair of false teeth to deal with the difficulties to chew food. Results The reproduction of mandibular denture in the mouth trials,the occlusion,reten- tion and extension of the maxillary mandibular edge were in good position, upper and lower midline para consistent, the opening and closing movement of mandibular protrusion of posterior teeth were sharp, staggered relative occlusal fossa. X ray examination showed that the morphology of the joint was not abnormal, and the condyle was located in the fovea. Conclusion Keeping fake teeth at upper jaw and modilying the four artificial teeth at lower jaw could restore beauty,functions of chewing, pronunciation and so on.
