宁胃消郁汤联合促胃肠动力、抑酸和抗躯体化症状疗法在功能性消化不良患者中的疗效研究

摘　　要：目的探讨宁胃消郁汤联合促胃肠动力、抑酸和抗躯体化症状疗法在功能性消化不良(FD)患者中的疗效。方法选取2015年2月至2016年11月在济源市中医院就诊的合并躯体化症状的FD患者116例,随机分为试验组(n=58)和对照组(n=58)。对照组给予促胃肠动力药物(枸橼酸莫沙必利)、抑酸药物(兰索拉唑)和抗躯体化症状药物(氟哌噻吨美利曲辛);试验组在此基础上加用自拟的宁胃消郁汤。比较两组治疗前后的各个症状评分、患者健康问卷躯体症状群量表(PHQ-15)、尼平消化不良指数(NDI)总分及临床疗效;并比较两组患者3、6个月的复发率。结果治疗后试验组各症状评分均低于对照组,差异有统计学意义(P<0.05)。试验组治疗后的PHQ-15、NDI总分均低于对照组,临床疗效明显优于对照组,差异有统计学意义(P<0.05)。试验组治疗后3、6个月的复发率分别为8.6%、12.1%,均低于对照组的22.4%、27.6%,差异均有统计学意义(P<0.05)。结论宁胃消郁汤联合促胃肠动力、抑酸和抗躯体化症状疗法对治疗FD有满意的疗效,可显著缓解患者的临床不适感,改善其生活质量,降低复发风险,值得临床推广。Objective To investigate the curative effect of Ningweixiaoyu decoction combined with the gastrointestinal prokinetic,acid inhibitory and anti-somatization therapy on patients with functional dyspepsia（FD）.Methods 116 patients with both FD and somatization symptoms treated in Jiyuan Hospital of Traditional Chinese Medicine from February of 2015 to November of 2016 were enrolled in the study and randomly divided into the observation group（n=58）and the control group（n=58）.The patients in the control group were given the conventional drug treatment with the gastrointestinal prokinetic agent Mosapride Citrate,the acid inhibitory drug Lansoprazole and the anti-somatization drug Flupentixol Melitracen,while those in the observation group were treated with more Ningweixiaoyu decoction on the basis of the conventional drug treatment.The scores of symptoms,the total scores of Patient Health Questionnaire-15（PHQ-15）and Nepean Dyspepsia Index（NDI）,and the clinical efficacy were compared between the two groups before and after treatment.The recurrence rates were also compared between the two groups after 3and 6 month of treatment respectively.Results The scores of all symptoms in the observation group were significantly lower than those in the control group（P〈0.05）after treatment.The total scores of PHQ-15 and NDI in the observation group were significantly lower than those in the control group（P〈0.05）,and the clinical efficacy in the observation group was significantly better than that in the control group（P〈0.05）.The recurrence rates after 3and 6 months of treatment in the observation group were8.6% and 12.1%respectively and significantly lower than those in the control group which were 22.4%and 27.6%respectively（P〈0.05）.
