血清前白蛋白与结肠癌患者预后的关系研究

摘　　要：目的:研究结肠癌患者术前血清前白蛋白水平的临床意义。方法:共有198名在我院行结肠癌手术的患者纳入该研究,以血清前白蛋白165 mg/L为标准将患者分为正常组和降低组,随访至2017年1月。随访期间收集患者的病理特征资料和生存资料。采用Kaplan-Meier法进行生存分析,并进一步采用COX比例风险模型进行生存资料多因素分析。结果:血清前白蛋白降低的患者有74名,占患者总数的37.8%。术前血清前白蛋白水平与肿瘤的TNM分期、浸润深度、有无淋巴结转移、脉管浸润以及肿瘤部位密切相关(P<0.05)。术前血清前白蛋白降低的患者预后更差(P=0.001),多因素COX回归多因素分析进一步证实术前血清白蛋白水平是结肠癌患者的独立预后指标(RR=3.243,95%CI=1.892~5.582)。结论:术前血清前白蛋白水平降低的结肠癌患者有着更差的临床病理情况及预后,可以作为判断患者预后的指标。Objective： To investigate the clinical significance of preoperative serum prealbumin level in patients with colon cancer. Methods： A total of 198 patients with colon cancer who underwent surgery in our hospital were enrolled, and according to a standard serum prealbumin level of 165 mg/L, the patients were divided into normal group and reduced group. All patients were followed up to January 2017. Pathological features and survival data were collected during follow-up. The Kaplan-Meier method was used for survival analysis, and the Cox proportional hazard model was used for multivariate analysis of survival data. Results： Of all patients, 74 （37.8%） had a reduction in serum prealbumin level. Preoperative serum prealbumin level was closely associated with TNM stage, depth of infiltration, presence or absence of lymph node metastasis, vessel invasion, and tumor location （P 〈 0.05）. Compared with those with a normal preoperative prealbumin level, the patients with a reduced level had a poorer prognosis （P = 0.001）. The multivariate Cox regression analysis showed that preoperative serum prealbumin level was an independent prognostic factor for patients with colon cancer （RR = 3.243, 95% CI = 1.892 - 5.582）. Conclusions： Colon cancer patients with a reduced preoperative serum prealbumin level tend to have poor clinicopathological conditions and prognosis, and preoperative serum prealbumin level can be used for prognostic evaluation.
