摘  要:目的:观察无机三氧化物聚合体(mineral trioxide aggregate,MTA)与氢氧化钙用于髓室底穿孔修补的临床疗效。方法:选取2012年6月至2014年12月口腔科门诊髓室底穿孔患牙40例,随机分为MTA组和Ca(OH)_2组两组,每6个月复查1次,经过6~24个月观察,通过X线片检查和临床效果评价疗效。结果:MTA组6个月的成功率与Ca(OH)_2组差异无统计学意义(χ~2=1.111,P=0.292)。MTA组12个月的成功率高于Ca(OH)_2组(χ~2=4.286,P=0.038),MTA组18个月成功率高于Ca(OH)_2组(χ~2=5.013,P=0.025);MTA组24个月的成功率高于Ca(OH)_2组(χ~2=8.120,P=0.004)。MTA组治疗成功率与穿孔原因有关,医源性穿孔治疗成功率高于龋源性穿孔,差异有统计学意义(χ~2=5.089,P=0.024)。结论:MTA具有良好的封闭性,适用于髓室底穿孔的修补,长期疗效较好。Objective:To observe the clinical effects of mineral trioxide aggregate (MTA) and Ca(OH) 2 on repairing the perforation of pulp floors. Methods : 40 cases of involved teeth from June 2012 to December 2014 in the Department of Stomatology were randomly divided into MTA group and Ca(OH) 2 group. Both were reviewed once in every six months. After six months to twenty -four months follow - up, clinical and X - ray effects were evaluated, Results: There was no difference of success rate between MTA group and Ca( OH)2 group in six months ( X^2 = 1.111, P = 0. 292). The success rate in MTA group was better than that in Ca( OH)2 group in 12 months (x^2 = 4. 286, P = 0. 038), the success rate in MTA group was better than that in Ca(OH) 2 group in 18 months (X^2 = 5. 013, P = 0. 025 ), and the success rate in MTA group was better than that in Ca(OH)2 group in 24 months (X^2 =8. 120, P =0.004). The success rate in MTA group was related with the perforation of pulp floor. The success rate of iatrogenic perforation was significantly higher than that with carious perforation ( X^2 = 5. 089, P = 0. 024 〈 0.05 ). Conclu- sion: MTA, with favorable sealing property, is a nice repairing material in the treatment of perforation of pulp floors and has a salutary effect in a long term.
