经腹腔行腹腔镜下肾输尿管全长膀胱袖状全封闭切除术的临床研究

摘　　要：目的探讨经腹腔行腹腔镜肾输尿管全长切除术联合下腹部小切口膀胱袖状切除术治疗上尿路尿路上皮癌(upper urinary tract urothelial carcinoma,UUT-UC)的临床研究。方法选取揭阳市人民医院泌尿外科2014-02—2016-05间诊治的16例尿路上皮癌患者临床资料进行回顾性分析,记录围手术期参数,包括手术时间,术中出血量,住院时间,引流管的拔除时间,术后开始进食时间,围手术期并发症,术后随访如膀胱、手术区域肿瘤复发的情况。结果术中术后未发生胃肠损伤、大出血、肺炎等并发症。平均随访16(8~22)个月,其中1例术后出现因膀胱肿瘤复发接受经尿道膀胱肿瘤电切术(trans urethralresection of bladder tumour,TURBT)。其他患者未发现肿瘤复发。结论经腹腔行腹腔镜肾输尿管全长切除术并下腹部小切口膀胱袖状切除术治疗上尿路尿路上皮癌是可行的、安全有效的。Objective To investigate the transperitoneal laparoscopic nephroureterectomy combined with abdominal incision bladder cuff resection in the treatment of upper urinary tract urothelial carcinoma（ UUT-UC）. Methods Retospective analysis of clinical data of 16 patients with upper urinery tract wrothelial carcinoma in Urinary Surgery of Jieyang People’s Hospital from Feb.2014 to May 2016 was done. Records of perioperative parameters were mad,including operation time,amount of bleeding during operation,hospitalization time,removal time of drainage tube,postoperative feeding time and perioperative complications,such as postoperative bladder or surgery regional recurrence. Results Postoperative gastrointestinal injury did not occur,neither did perioperative hemorrhage,pneumonia and other complications. The average follow-up was 16（ 8 ~ 22） months. One of the patients had the recurrence of bladder cancer and had transurethral resection of bladder tumor（ TURBT） to undergo. The other patients had no recurrence. Conclusion Transperitoneal laparoscopic nephroureterectomy and abdominal incision of bladder cuff resection in the treatment of upper urinary tract urothelial carcinoma is feasible,safe and effective.
